Traumatic brain injury, missing app access (no phone or tablet), aphasia with strong difficulties in understanding language (e.g. global aphasia, Wernicke aphasia or transcortical sensory aphasia), strong cognitive or neuro-psychological deficits, paresis of both arms or ataxia of both arms or strong ideomotor or ideatory apraxia of both arms.

- Primary Outcome Measures
Name Time Method Change in post-stroke depression (PHQ-2) & activity and participation (IMET), adherence to therapy and adherence to medication (MMAS-8), Health Literacy (HLS-EU-Q16), Self-Efficacy (ASKU), Health Related Quality of Life (SOEP-SF12v2) and amount of recurrent strokes. All measured at the individual starting point (T0) and after three months (T1).
