Inclusion Criteria

ICD I60-64, ICD I67, ICD I69, ICD G45, capability to understand provided information and to use the app,
german language skills,
Barthel-Index >30,
informed consent

Exclusion Criteria

Traumatic brain injury, missing app access (no phone or tablet), aphasia with strong difficulties in understanding language (e.g. global aphasia, Wernicke aphasia or transcortical sensory aphasia), strong cognitive or neuro-psychological deficits, paresis of both arms or ataxia of both arms or strong ideomotor or ideatory apraxia of both arms.

Primary Outcome Measures

Change in post-stroke depression (PHQ-2) & activity and participation (IMET), adherence to therapy and adherence to medication (MMAS-8), Health Literacy (HLS-EU-Q16), Self-Efficacy (ASKU), Health Related Quality of Life (SOEP-SF12v2) and amount of recurrent strokes. All measured at the individual starting point (T0) and after three months (T1).
