Eligibility Criteria

Inclusion Criteria

  • (1\) Remained relatives of patients deceased in the observation period of the involved general practices:
  • Age \> 18, close associate and in direct contact with deceased patient in the last phase of life, (independent of sex)
  • (2\) Deceased patients of targeted general practices:
  • Age: \> 70 (independent of sex)
  • Diagnosed life\-limiting, chronic oncological and non\-oncological disease(s) and at least two indicators of the Supportive and Palliative Care Indicators Tools (SPICT) applied \[https://www.spict.org.uk/]; deceased within the 6 month observation period pre or post intervention respectively.
  • (3\) Health Professionals
  • a. Key representatives/stakeholder in the regional for older patients:
  • Owing their position/being employed at for least 1year in the institution (e.g., community nursing agency, nursing home, hospice) (independent of sex)
  • b. General Practitioners:
  • Registered in the region and at least one year of professional experiences in the respective general practice (independent of sex)

Exclusion Criteria

  • (1\) Bereaved relatives of deceased older patients of the targeted general practices:
  • Sudden death of the deceased patient, not related to a life\-limiting chronic disease (e.g. traffic accident, heart attack without diagnosis of coronary heart disease), complicated grief and/or mental illness
  • (2\) Deceased older patents of the general practices:
  • Sudden death, not related to a life\-limiting chronic disease (e.g. traffic accident, heart attack without diagnosis of coronary heart disease)
