Acute febrile illness (fever >38.3?C for more than 48 h duration and onset <14 days) without focus and with serological evidence viz IgM for leptospira,NS1/IgM for Dengue, weil felix/IgM for Scrub typhus,IgM for Brucella,anti-HAV IgM & HEV IgM for hepatitis A &E ,peripheral smear for malaria &blood culture for enteric fever & meliodosis
1.Patients already treated in an outside ICU for more than 72 hrs.
2.Patient with immunodeficiency state (congenital oracquired); on immune-suppressants therapy (oral corticosteroids, combination chemotherapy); and with autoimmune disorders, malignancies or organ transplants.
3.Patients with pre-existing chronic conditions- chronic liver disease, chronic kidney disease, malignancy.
4. Anemia of chronic disease due to any cause

- Primary Outcome Measures
Name Time Method Primary outcome

To assess association between serum ferritin levels in tropical fever patients measured within 72 hrs of ICU admission to ICU mortality & ICU length of stay .

Timepoint: Until icu stay or death

- Secondary Outcome Measures
Name Time Method Secondary outcomes

1.To assess the incidence of secondary HLH in tropical infections admitted to ICU .

2.To assess the association between secondary HLH & ICU mortality & ICU length of stay.

3.To assess the mortality difference among patients treated with & without steroids based on serum ferritin levels alone.

Timepoint: until icu stay
