All patients with an acute exacerbation COPD (AECOPD) admitted to the hospital, and

- Aged 18 years or older.

Exclusion Criteria

- Not speaking the dutch language, or

- Severe cognitive disorders, or

The primary outcome measure of the indicator study is: <br /><br>- Length of time from the moment that a patient hospitalized for AECOPD meets two or more criteria of the set of indicators until death for any cause.<br /><br>The primary outcome measure of the intervention study is:<br /><br>- Quality of life of the patient (SGRQ)<br>
