- •receiving vancomycin when being admitted in KCMH
- •consent to participate in the project
排除标准
- •renal failure or abnormal serum Cr, depends on age
- •receiving renal replacement therapy or ECMO
- •receiving amphotericin B, cyclosporin, tacrolimus
