Patients with at least one anticonvulsant medication prescribed.
Exclusion Criteria
Patients that are fostered in a nursing home or served by a nursing service. Patients with an official guardian.

Name Time Method Patients/processes with at least one error in drug handling 3 to 6 weeks after hospital discharge or outpatient visit. Data assessment at home visits by process monitoring of a clinical pharmacist.
