Patients over the age of 18 years old who had been go on flxible bronchoscope for bronchoalveolar lavage or transbronchial biopsy in the medicine department, Vajira Hospital, Navamindradhiraj University and had conditions follwing criteria :
1 patients who had inform consent
2 can communicated and assessed outcome after bronchoscope
3 stable vital sign before bronchoscope defined as heart rate less than 100 beats/min, blood pressure less than 150/90 mmHg
1 Patients who go no bronchoscpoe with cryobiopsy
2 Baseline PaCO2 more than 45 mmHg
3. long term Oxygen therapy patients
4. have an upper airway obstruction or foreign body
5 current on antiplatelet or anticoagulant or withdrawal less than 5 days
6 Emergency bedside bronchoscope
7 Had head and neck surgery within 4 weeks
8 had a history of asthmatic attack, COPD with AE, or respiratory infection within 2 weeks
9 Had uncontroll allergic rhinitis or anomaly of head and neck
