Inclusion Criteria

patients who underwent simultaneous unilateral and bilateral total knee arthroplasty (TKA)

Exclusion Criteria

Contraindication for total knee arthroplasty from Thai OA knee guideline 2011
contra-indication for spinal anesthesia
cognitive impairment patient
Chronic renal disease (Creatinine clearance < 30 ml/min)
Patients who are chronic alcohol users (defined as >1 drink per night for women or >2 drinks per night for men)
Patients with poorly controlled diabetes mellitus (HbA1C >7.5 on preadmission testing)
Patients with steroid or immunosuppressive drug use within 6 months of surgery
Severe heart and liver disease
Preoperative history of venous thromboembolism or deep vein thrombosis
Patients with a hypercoagulable disorder
Patients with a concurrent physical condition that may require analgesic treatment during study follow-up
Allergy to drugs in the study

Primary Outcome Measures

Visual analog pain score 4, 24, 48 hours, 2 weeks and 6 weeks postoperatively point/ ask patient give pain score in scale 0-10 point while they rest and on motion

extra dose opioid 24, 48 hours postoperatively miligram/ record from PCA chart,Time to first dose opioid 24 hours hours/ time zero is immediate post op to time that patient request IV opioid (while pain score more than 3),Length of stay 5 days days /time from admission to discharge,modified Thai WOMAC score 1 day before operation, 2 and 6 weeks after operation point/ ask patient and record in modified Thai WOMAC score chart then sum total score,Range of motion 1 day before operation, 24,48 hours, 2 and 6 weeks after operation degree/ used goniometer to measure range of motion from maximum extension to flexion
