Name Time Method Proportion of RPH ACS patients receiving cardiac rehabilitation and secondary prevention inital assessment and individualised plan (recorded in hospital CR database). [at 2 weeks post discharge<br><br>]; Proportion of patients receiving follow up at 3 months post ACS event to assess clinical management plan in place. <br>Hospital databases and patient self report utlised. [at 3 months post discharge];Cost effectiveness of CR post ACS admission. Hospital and staff costs measured using hospital databases. [12 months]
