1. Meet the diagnostic criteria of the five kinds of chronic diseases.The diagnosis of angina refer to the Guidelines for the diagnosis and treatment of unstable Angina and non-ST-elevation myocardial infarction;the diagnosis of Myocardial infarction refer to the Guidelines for integrated traditional and Western medicine in the diagnosis and treatment of acute myocardial infarction;the diagnosis of heart failure refer to the Guidelines for the diagnosis and treatment of heart failure in China;the diagnosis of Cerebral infarction refer to Chinese guide to diagnosis and treatment of cerebral infarction with integrated traditional Chinese and Western medicine;the diagnosis of Cerebral hemorrhage refer to Guidelines for the treatment of cerebral hemorrhage in China;
2. Aged over 18 years;
3. Signing the informed consent.
1. Patients with severe mental illness;
2. Patients with severe cognitive impairment,serious communication deficiencies,or unable to match the researchers to complete the study;
3. Participated in other drug clinical trials or other study of chronic disease management now.
