Inclusion Criteria

Patients above 18 years of age, who are inpatients on Siebeck ward, are cognitively, linguistically as well as motorically suitable, and have at least completed the screening questionnaires.

Proportion of patients with intervention reporting drug therapy problems during follow-up phone calls compared to patients who had no intervention and complained of drug therapy problems during follow-up phone calls.
Secondary Outcome Measures

- Type and number of drug therapy problems reported during follow-up calls.<br>- Outcome of the screening questionnaires with regard to a detected need for intervention and possible associations with the actual needs of the patient in the intervention consultation or follow-up phone call<br>- Content and course of the interventions<br>- Analysis of potential associations between screening, identification of a need, course of the intervention and information given in follow-up calls, possibly in relation to other patient characteristics (e.g., age, gender, diagnosis, number and type of drugs, length of hospital stay, other scores collected during routine care).<br>- Proportion of patients reached with the interventions compared to all patients treated on the ward, including evaluation of the group representativeness.
