Inclusion Criteria
Patients above 18 years of age, who are inpatients on Siebeck ward, are cognitively, linguistically as well as motorically suitable, and have at least completed the screening questionnaires.

- Primary Outcome Measures
Name Time Method Proportion of patients with intervention reporting drug therapy problems during follow-up phone calls compared to patients who had no intervention and complained of drug therapy problems during follow-up phone calls.
- Secondary Outcome Measures
Name Time Method - Type and number of drug therapy problems reported during follow-up calls.<br>- Outcome of the screening questionnaires with regard to a detected need for intervention and possible associations with the actual needs of the patient in the intervention consultation or follow-up phone call<br>- Content and course of the interventions<br>- Analysis of potential associations between screening, identification of a need, course of the intervention and information given in follow-up calls, possibly in relation to other patient characteristics (e.g., age, gender, diagnosis, number and type of drugs, length of hospital stay, other scores collected during routine care).<br>- Proportion of patients reached with the interventions compared to all patients treated on the ward, including evaluation of the group representativeness.
