RHD patients with symptomatic long standing persistent atrial fibrillation in the age group of >= 18years with LA diameter of more than 4.5 cm in heart failure with NYHA functional class >= II, despite optimal medical therapy will be included in the study.
1-Severe valvular lesions requiring surgery or intervention
2- Significant Coronary artery disease
3-Ongoing infection
4-Pregnancy
5-Severe co-morbidies limiting life expectancy
6-Untreated hypothyroidism, hyperthyroidism
7-Mental or physical inability to participate in the study
