Inclusion Criteria
1. understands the study purpose and design
2. contractually capable and provides signed informed consent form
3. admitted into specialized geriatric rehabilitation after an acute event
Exclusion Criteria
1. admitted for acute surgical treatment
2. discharge before completion of the 2-week geriatric rehabilitation program

- Primary Outcome Measures
Name Time Method Changes in medication between admisson into the acute geriatric rehabilitation program and at dischcharge to determine the prevalence of polypharmacy and use of PIM. The medication plans will be collected from the doctor's letter.
- Secondary Outcome Measures
Name Time Method 1. Adverse events during hospital stay that are possibly linked to PIM use<br>2. renal function changes with eGFR using the CKD-EPI formula and other renal<br>parameters (e.g. creatinine, potassium, sodium)<br>3. changes in liver function tests during hospital stay<br>4. changes in blood count during hospital stay
