Patient age = 18 years (with atrial fibrillation, history of deep vein thrombosis, and left atrial enlargement).
- Long-term therapy with a direct oral anticoagulant (Rivaroxaban, Apixaban, Dabigatran, or Edoxaban).
- Elective urological operation.
- Mechanical heart valve and/or use of vitamin K antagonists.
- Patients with GFR (glomerular filtration rate) < 30 ml/min (Dabigatran).
- Patients with GFR < 15 ml/min (Apixaban, Rivaroxaban).
- Ongoing dual antiplatelet therapy (ADP receptor blocker, P2Y12).
- Patients planned for cystectomy with urinary diversion.
- Cognitive limitations or psychiatric conditions preventing adherence to the study protocol.
