Ovarian cancer (search) treatment has advanced considerably over the past decade, with new drugs and immune-based therapies improving outcomes for many patients. Yet the disease remains notoriously difficult to treat: many patients experience recurrence, and over time tumors can stop responding to chemotherapy. "We need better options," explains gynecologic oncologist Emese Zsiros, MD, PhD, FACOG, of Roswell Park Comprehensive Cancer Center. "Clinical trials are how we move the field forward and find better treatments for patients."
The message from experts is clear—clinical trials are not a last resort. "Some of the most important clinical trials are offered early in care, even at the time of diagnosis," says Dr. Zsiros. "These trials test new ways to make standard treatments work better."
Access to Trials at Every Stage of Disease
Roswell Park, a National Cancer Institute-designated comprehensive cancer center and the only one in New York State outside of New York City, offers patients access to numerous clinical trials spanning all stages of ovarian cancer (search) treatment. When meeting with a patient, the care team reviews all appropriate treatment options, including both standard therapies and any clinical trials that may be a good fit. "Our goal is to make sure patients understand every available option so they can make informed decisions about their care," Dr. Zsiros notes.
One current trial at Roswell Park takes a novel approach by asking patients to wear a fitness tracker, with researchers investigating whether increased physical activity can affect the gut microbiome and immune function during ovarian cancer (search) treatment.
A Patient's Perspective: "Future-Oriented Hope"
Orsolina Sepe, a now-retired psychiatrist, enrolled in a clinical trial after undergoing surgery and chemotherapy for her ovarian cancer (search). While standard treatments initially appeared effective, her bloodwork within months showed that a key biomarker was increasing—a sign of recurrence. "The clinical trial gave me future-oriented hope amidst the scientific reality of what was going on," she recalls. "I knew ovarian cancer was notoriously difficult and that many women do experience a recurrence. Of course, I was terrified and anxious, but it brought me to the point of wanting to do something about it."
The trial she joined involved an existing immunotherapy that was already FDA-approved for other cancer types, assessing its efficacy in ovarian cancer (search) patients. The drug received approval for use in ovarian cancer earlier this year. "For me, this research was everything," says Orsolina. "It matters so much for me and for other patients with ovarian cancer."
Novel Therapeutic Strategies Under Investigation
Roswell Park is currently advancing several cutting-edge treatment modalities for ovarian cancer (search). CAR T-cell therapy, which uses a patient's own immune cells modified in the laboratory to better fight cancer, has demonstrated success in certain blood cancers and is now being studied in a new version for ovarian cancer. Next-generation antibody drug conjugates—described as "smart missiles"—attach to cancer cells and deliver medicine directly into the tumor, potentially limiting damage to healthy tissue. Additionally, new PARP inhibitors are being explored; these agents block a process that cancer cells use to repair themselves, which can slow or stop cancer growth and may enhance the effectiveness of other treatments.
The Essential Role of Patient Participation
"Clinical trials are an important part of modern cancer care," emphasizes Dr. Zsiros. "Patients should feel comfortable asking their physicians about them at any time. Clinical trials are not a last resort—they are often the pathway to the next generation of cancer treatments."
This sentiment is echoed by the broader oncology community. Every treatment used today was once only available through a clinical trial, underscoring the indispensable role that patient participation plays in advancing the standard of care.
