EMA Issues Precautionary Measures for Valproate Use in Male Patients Due to Potential Neurodevelopmental Risks

The European Medicines Agency (search)'s Pharmacovigilance Risk Assessment Committee (search) (PRAC) has recommended precautionary measures for male patients taking valproate medicines, addressing a potential increased risk of neurodevelopmental disorders (search) in children conceived by fathers using the medication.
The recommendation follows a review of data from a retrospective observational study conducted across Denmark, Norway, and Sweden, which compared birth outcomes in children born to men taking valproate versus those taking alternative epilepsy (search) treatments such as lamotrigine or levetiracetam.
Study Findings and Risk Assessment
The Nordic registry study results suggest that approximately 5 out of 100 children born to fathers treated with valproate in the three months before conception developed neurodevelopmental disorders (search), compared to around 3 out of 100 children born to fathers taking lamotrigine or levetiracetam.
Neurodevelopmental disorders (search) identified in the study include autism spectrum disorders (search), intellectual disability (search), communication disorders, attention deficit/hyperactivity disorders (search), and movement disorders that begin in early childhood.
The PRAC notes that this potential risk is significantly lower than the previously confirmed risk in children born to women who take valproate during pregnancy, where up to 30-40% of preschool children may experience developmental problems.
Dr. Sabine Straus, Chair of the PRAC, stated: "While the study has important limitations that prevent us from establishing definitive causation, we believe precautionary measures are warranted to inform healthcare professionals and patients about this potential risk."
Recommended Precautionary Measures
Based on their assessment, the PRAC has recommended several precautionary measures:
For Healthcare Professionals:
- Valproate treatment in male patients should be initiated and supervised by specialists in epilepsy, bipolar disorder, or migraine management
- Regular reviews of valproate treatment to assess if it remains the most appropriate option
- Discussion of effective contraception with male patients and their female partners during treatment and for at least three months after discontinuation
- Male patients should be advised not to donate sperm during treatment and for three months afterward
For Male Patients:
- Awareness of the potential risk to children conceived while taking valproate
- Consideration of effective contraception during treatment and for three months after stopping
- Consultation with healthcare providers before planning conception
- Not stopping treatment without medical consultation, as symptoms may worsen
Study Limitations and Context
The PRAC identified several limitations in the study data, including differences between patient groups in the conditions being treated and follow-up times. Additionally, the study size was insufficient to identify specific types of neurodevelopmental disorders (search) that might be associated with paternal valproate exposure.
"Despite these limitations, we believe a precautionary approach is appropriate while further research is conducted," explained a spokesperson for the EMA.
Valproate Background and Current Use
Valproate medicines are widely prescribed to treat epilepsy (search), bipolar disorders and, in some EU countries, migraine (search). The active ingredient may appear as valproic acid, magnesium valproate, sodium valproate, valproate semisodium, or valpromide, marketed under various brand names including Depakine, Epilim, and Orfiril.
Previous EMA recommendations already restrict valproate use in women of childbearing potential due to well-established risks of congenital malformations and neurodevelopmental disorders (search) in children exposed during pregnancy. These restrictions remain firmly in place.
Implementation Timeline
The potential risk of neurodevelopmental disorders (search) and the precautionary measures will be reflected in updates to product information and educational materials for valproate medicines. A direct healthcare professional communication will be distributed to prescribers, and patient guides and cards will be updated to include this new information.
Male patients currently taking valproate are advised not to discontinue treatment without consulting their healthcare provider but should discuss these new precautionary measures at their next appointment.
The PRAC recommendations will now be sent to the Co-ordination Group for Mutual Recognition and Decentralised Procedures – Human (CMDh), which will adopt a position to ensure harmonized safety standards across the EU.
