FDA Approves Datroway as First TROP2-Directed ADC for First-Line Triple-Negative Breast Cancer Treatment

AstraZeneca and Daiichi Sankyo have secured FDA approval for Datroway (datopotamab deruxtecan) to treat metastatic triple-negative breast cancer (search) (TNBC (search)), marking the first approval of a TROP2 (search)-directed antibody drug conjugate (ADC) in this patient population. The approval allows Datroway to be used as a first-line treatment for adults with unresectable or metastatic TNBC who cannot receive programmed cell death protein 1 (PD-1 (search)) or PD-L1 (search) inhibitor therapy.
Addressing Critical Unmet Medical Need
The approval addresses a significant gap in treatment options for patients with the most aggressive form of breast cancer (search). TNBC (search) accounts for 15% of all breast cancer cases and has one of the worst prognoses, with a median overall survival of just 12 to 18 months. According to the Triple Negative Breast Cancer Foundation (search), seven out of 10 patients with metastatic TNBC are not candidates for immunotherapy, leaving chemotherapy as the only treatment option.
"Today's approval of Datroway means that for the first time, these patients will have a new standard of care beyond traditional chemotherapy at the outset of their treatment," said Arlene Bothers, executive director of the Triple Negative Breast Cancer Foundation (search).
Pivotal Trial Results
The FDA granted priority review for Datroway based on data from the TROPION-Breast02 Phase III trial. In this study, Datroway achieved a statistically significant five-month improvement in median overall survival compared to chemotherapy, becoming the first drug in the TROP2 (search) class to improve overall survival in first-line TNBC (search) patients. The trial showed an "unprecedented" median overall survival of approximately two years.
The study also demonstrated a 43% reduction in patients' risk of disease progression or death and an objective response rate of 64% compared to 30% with chemotherapy. The safety profile of Datroway matched that of earlier breast cancer (search) trials.
Commercial Potential and Market Position
Datroway was previously approved by the FDA for previously treated HR-positive, HER2-negative breast cancer (search) and EGFR-mutated non-small cell lung cancer (search). The drug has seen accelerating sales, bringing in $218 million last year for AstraZeneca and Daiichi Sankyo, with another $102 million in the first three months of 2026. AstraZeneca has predicted the drug could eventually reach $5 billion in peak annual sales across all its indications.
The approval positions Datroway ahead of Gilead Sciences' rival TROP2 (search) ADC Trodelvy (sacituzumab govitecan), which has positive progression-free survival data in the first-line setting from the ASCENT-03 study but has not yet demonstrated an overall survival benefit in this population.
Global Regulatory Strategy
The application was reviewed under Project Orbis, which allows for synchronized submissions to health authorities in Canada, Australia, Switzerland, and Singapore. Reviews continue in Japan, China, and the European Union.
Future Development Plans
AstraZeneca and Daiichi Sankyo are planning to extend the label for Datroway even further in TNBC (search) through multiple ongoing studies. These include the TROPION-Breast05 study in PD-1 (search)/PD-L1 (search)-eligible patients, with or without AstraZeneca's PD-L1 inhibitor Imfinzi (durvalumab), as well as TROPION-Breast04 and TROPION-Breast03 in neoadjuvant and post-neoadjuvant treatment of early TNBC.
