"There is an important caveat to our findings - the window for the FMT intervention is very narrow because these patients are generally extremely sick," said Dr. Alexander Khoruts, professor at the University of Minnesota Medical School and director of the UMN Microbiota Therapeutics Program. "Therefore, the FMT formulation needs to be easily accessible."
Regulatory Changes Create Access Challenges

The regulatory changes have particularly impacted pediatric patients, as illustrated by the case of Mundo, a 7-year-old from Las Vegas who developed recurrent C. difficile infection (search) following antibiotic treatment for strep throat (search). After six hospitalizations and six courses of antibiotics (search) over eight months, his family sought FMT treatment.
"It shouldn't be this hard to cure a child," said Blanca Morales, Mundo's mother, who traveled to Los Angeles seeking treatment only to learn that her son was too young for the available clinical trial protocols.
When doctors prescribed off-label Vowst for Mundo, the family faced multiple insurance denials before ultimately receiving approval. The case highlights the complex approval process for off-label use of these expensive medications in pediatric populations.
Clinical Impact and Future Directions
C. difficile infection (search) represents one of the most urgent infectious disease threats according to the Centers for Disease Control and Prevention, with an estimated 15,000 annual deaths in the United States. The infection typically occurs in patients with disrupted gut microbial communities (search), most commonly following antibiotic treatment.

The OpenBiome (search) Foundation has submitted a research protocol to the FDA seeking approval to resume distribution, though the timeline for regulatory decision remains unclear. The agency responded by the March 13 deadline only to indicate it needed more time for evaluation.
"FDA recognizes that microbiome-based products are being studied for a variety of diseases, conditions and populations. We continue to work with companies to advance the development of their products," an FDA spokesperson stated.
Meanwhile, some institutions continue to provide in-house FMT preparation, though this requires significant infrastructure and expertise. The University of Minnesota maintains its own pharmaceutical-grade manufacturing facility and cryobank, enabling continued access for qualifying patients.
