The treatment was administered in late October under the direction of medical oncologist Dr. Zhonglin Hao, marking the opening of the first U.S. clinical trial for this novel therapeutic approach.
Enhanced Immunotherapy Design
Alveltamig (搜索) represents an advancement in T-cell engager technology, featuring a trispecific design that distinguishes it from currently available treatments. While Imdelltra (tarlatamab), approved by the FDA in 2024 for small cell lung cancer patients whose disease worsened after chemotherapy and immunotherapy, operates as a bispecific T-cell engager with two binding arms, Alveltamig incorporates three arms—two that attach more tightly to cancer cells and one that connects to T cells.
"This is the next step forward in T-cell engager therapy for small cell lung cancer," said Dr. Hao, who serves as lead investigator for the trial, co-director of Markey's Thoracic Oncology Program and medical director of Markey's Clinical Research Office. "We're testing a newer version of tarlatamab that has a stronger ability to connect cancer cells with immune cells. This trial will explore whether that enhanced design can help more patients respond to treatment or give those who've tried tarlatamab another option."
Early Safety and Tolerability Results
Initial treatment experience has demonstrated encouraging tolerability profiles. The first patient treated with Alveltamig (搜索) experienced no side effects from the initial dose and returned two weeks later for a second treatment that was also well-tolerated. Patients enrolled in the trial are being closely monitored for potential side effects as dosing continues.
The therapy shows particular promise for addressing brain metastases, a common complication in small cell lung cancer that typically requires whole-brain radiation therapy. According to Dr. Hao, the immunotherapy may have potential activity in brain metastases, and by improving control of disease throughout the body, could reduce or eliminate the need for brain radiation for some patients. This represents a significant potential benefit, as whole-brain radiation therapy is associated with side effects including fatigue, hair loss, and cognitive problems.
Addressing Unmet Medical Needs
The trial addresses critical treatment gaps for patients with small cell lung cancer, which is often diagnosed at advanced stages. While patients may initially respond to chemotherapy and radiation, the cancer frequently returns, creating a need for additional treatment approaches when standard options are no longer effective.
Phase 1 Trial Design and Expansion
The Phase 1 clinical trial represents the first U.S.-based study of Alveltamig (搜索), following an earlier trial conducted in China that demonstrated the treatment was well-tolerated among patients. The U.S. trial plans to enroll patients at six sites nationwide, with Markey Cancer Center serving as the inaugural site. Additional centers, including Case Comprehensive Cancer Center, are preparing to begin enrollment.
Markey's selection as the first U.S. site stems from its designation as a National Cancer Institute-designated Comprehensive Cancer Center, one of 57 in the country. This designation enables the center to offer access to clinical trials studying new treatments before they become widely available.
"We want Kentuckians to be among the first to benefit from this newer drug," Dr. Hao said. "Clinical trials represent the future of cancer care. When patients participate in trials at Markey, they're getting access to tomorrow's treatments today."
The phase 1 design focuses on closely evaluating patient tolerance while researchers gather early information about the drug's behavior in the body. This careful monitoring approach is particularly important for immunotherapies that engage the immune system through novel mechanisms. Information collected during this phase will guide future studies of Alveltamig (搜索) in small cell lung cancer and determine next steps for ongoing clinical development.
