Green Space Exposure During Pregnancy and Early Childhood Reduces Risk of Neurodevelopmental Disorders

The research, published in Environment International, analyzed data from approximately 1.85 million mother-child pairs identified from the Medicaid Analytic Extract between 2001 and 2014, with follow-up periods extending up to 14 years. The study represents one of the largest investigations to date examining the relationship between residential green space exposure and neurodevelopmental outcomes.
Timing-Specific Protective Effects
The study identified distinct protective associations based on the timing of green space exposure. Per interquartile range increase in the Normalized Difference Vegetation Index, the strongest associations were observed for preconception exposure and intellectual disability (search), with a hazard ratio of 0.66, representing a 34% reduction in risk. Pregnancy exposure showed the greatest benefit for autism spectrum disorder (search) prevention, with a hazard ratio of 0.83 (17% risk reduction), while postnatal exposure demonstrated the strongest protective effect against learning difficulties (search), with a hazard ratio of 0.81 (19% risk reduction).
"We observed protective associations between residential green space and several neurodevelopmental outcomes across distinct exposure windows – preconception, prenatal and early childhood – suggesting the involvement of different underlying biological mechanisms," said Dr. Stefania Papatheodorou, senior author and associate professor at the Rutgers University School of Public Health (search).
Enhanced Benefits for Vulnerable Populations
The protective effects were particularly pronounced among specific demographic groups. Black and Hispanic children showed stronger associations with green space exposure compared to other populations, as did children living in urban environments where green space access is typically more limited.
"Associations were more pronounced among children living in urban areas, suggesting a potentially greater benefit of green space where it is limited," Papatheodorou explained. "Our findings suggest that enhancing green space access in urban environments may support early childhood neurodevelopment and help reduce the burden of neurodevelopmental delays."
Proposed Mechanisms of Action
While the study establishes strong associations between green space exposure and reduced neurodevelopmental disorder risk, researchers have proposed several mechanisms that may explain these protective effects. Green space exposure is thought to reduce stress and depression in pregnant women and families, increase opportunities for social engagement, and lower exposure to environmental stressors such as noise pollution and extreme temperatures.
These factors collectively may create more favorable conditions for healthy brain development during critical prenatal and early childhood periods, though the exact biological pathways remain to be fully elucidated.
Clinical and Policy Implications
The findings suggest that urban planning strategies focused on increasing residential green space access could serve as a population-level intervention to reduce neurodevelopmental disorder risk, particularly in low-income and urban communities where such conditions are often more prevalent.
"These findings suggest that increasing green space access could be a potentially modifiable environmental strategy to reduce the risk of neurodevelopmental disorders (search) among children, especially in vulnerable, low-income populations," said Dr. Hayon Michelle Choi from the Harvard T.H. Chan School of Public Health, the study's lead author. "It also suggests that urban planning strategies that enhance residential greenness may have long-term developmental benefits for children."
The researchers acknowledge that while their study demonstrates strong associations, it cannot establish direct causation between green space exposure and neurodevelopmental outcomes. Future research will focus on identifying the specific biological mechanisms underlying these protective effects and determining whether different types of green spaces – such as parks, walking trails, or playing fields – provide varying levels of benefit for child neurodevelopment.
