Metabolic Surgery Demonstrates Superior Long-Term Outcomes Over GLP-1 Receptor Agonists in Diabetes and Obesity

A comprehensive Cleveland Clinic study has demonstrated that metabolic surgery provides superior long-term health outcomes compared to GLP-1 receptor (search) agonist medications for patients with type 2 diabetes (search) and obesity (search). The research, published in Nature Medicine, followed 3,932 adults for up to 10 years and found significant advantages in mortality, cardiovascular health, and diabetic complications for surgical patients.
Study Design and Patient Population
The M6 study (Macrovascular and Microvascular Morbidity and Mortality after Metabolic Surgery versus Medicines) compared outcomes between 1,657 patients (65.7% female) who underwent metabolic surgery and 2,275 patients (53.5% female) who received GLP-1 receptor (search) agonist treatment. The surgical procedures included gastric bypass and sleeve gastrectomy, while the medication group received various GLP-1 RAs including liraglutide, dulaglutide, exenatide, semaglutide, and tirzepatide.
Using a doubly robust estimation method to balance baseline characteristics between groups, researchers examined outcomes over a median follow-up of 5.9 years, tracking all-cause mortality, major adverse cardiovascular events (search) (MACE), nephropathy (search), and retinopathy (search).
Mortality and Cardiovascular Benefits
The study revealed striking differences in survival rates between treatment groups. The 10-year cumulative incidence of all-cause mortality was 9.0% (95% CI 6.8–10.8%) in the metabolic surgery group compared to 12.4% (95% CI 9.9–15.2%) in the GLP-1 RA group, representing a 32% lower risk of death (adjusted HR 0.68; 95% CI 0.48–0.96; P = 0.028).
Cardiovascular outcomes also favored surgical intervention. Metabolic surgery was associated with a 35% lower risk of major adverse cardiovascular events (search), including heart attack, heart failure, and stroke (adjusted HR 0.65; 95% CI 0.51–0.82; P < 0.001).
Microvascular Complications
The advantages of metabolic surgery extended to diabetic complications affecting smaller blood vessels. Patients who underwent surgery experienced a 47% lower risk of nephropathy (search) (adjusted HR 0.53; 95% CI 0.43–0.67; P < 0.001) and a 54% lower risk of retinopathy (search) (adjusted HR 0.46; 95% CI 0.29–0.75; P = 0.002) compared to those treated with GLP-1 receptor agonists.
Weight Loss and Glycemic Control
Metabolic surgery demonstrated superior effectiveness in achieving sustained weight reduction and glycemic control. Patients who underwent surgery lost an average of 21.6% of their body weight over 10 years, compared to 6.8% weight loss in the GLP-1 medication group.
Hemoglobin A1c levels, a key marker of long-term blood sugar control, improved significantly more with surgery (-0.86%) than with GLP-1 medicines (-0.23%). Additionally, patients in the surgery group required fewer prescriptions for diabetes, blood pressure, and cholesterol medications.
Clinical Implications
"Even with today's best medicines, metabolic surgery offers unique and lasting benefits for people with obesity (search) and diabetes," said Ali Aminian, M.D., director of Cleveland Clinic's Bariatric & Metabolic Institute and primary investigator of the study. "The benefits we observed went beyond weight loss. Surgery was linked to fewer heart problems, less kidney disease, and even lower rates of diabetes-related eye damage."
Steven Nissen, M.D., Chief Academic Officer of the Heart, Vascular & Thoracic Institute at Cleveland Clinic and senior author of the study, emphasized the significance of these findings: "Even in the era of these powerful new drugs to treat obesity (search) and diabetes, metabolic surgery may provide additional benefits, including a survival advantage."
Study Limitations and Future Directions
The researchers acknowledged several limitations of their observational study design. The research was not a randomized comparison of drugs and surgery, and it did not focus exclusively on the newest and most effective GLP-1 medicines. The authors noted that many patients discontinue GLP-1 medications over time, which may contribute to the observed differences in long-term outcomes.
Future studies should compare the cardiometabolic outcomes of metabolic surgery with newer GLP-1 RAs that are more effective for weight reduction, such as semaglutide and tirzepatide, to further guide treatment decisions for patients with obesity (search) and type 2 diabetes (search).
