- •70 years and older;
- •ISAR score> 2;
- •Taking 5 or more medications;
- •Pre-medication known;
- •Main and other diagnoses known;
- •Medication analysis performed:
- •- Postoperative (control group)
- •- pre- and postoperative (intervention group);
- •Preparation and transmission of treatment recommendations pre-operative (applies only to the intervention group);
- •at least one completed medication analysis including findings as well as the preparation and passing on of therapy recommendations;
- •Post-operative Medication analysis and diagnosis of discharge medication;
- •consent form

- •<5 medicines;
- •ISAR score <3;
- •Premedication unknown;
- •Main and secondary diagnoses unknown;
