入选标准
- •1. Patients aged 20 and above.
- •2. Clinical diagnosis of diabetes taking oral medication and having HbA1c more than or equal 7.5 percent.
- •3. Clinical diagnosis of hypertension and taking oral medication and having blood pressure more than or equal to 140/90 mmHg.
- •4. Clinical diagnosis of renal disease and an estimated glomerular filtration rate (eGFR) between 30 - 59 ml/min/ 1.73 m2 calculated based on the CKD Epidemiology Collaboration (CKD EPI) formula.
- •5. Able to read, write, and speak in Thai language.
- •6. Able to access the internet and mobile applications.
排除标准
- •1. Patients on dialysis or death.
- •2. Participating in other intervention.
- •3. Terminal illness other than CKD such as cancer or severe heart failure.
