This randomized controlled trial aims to evaluate the effects of nurse-led branching scenario-based education delivered through virtual reality and tablet on cardiovascular disease risk awareness and adherence to antiplatelet medication in patients with coronary artery disease. Participants will be randomly assigned to a virtual reality education group, a tablet-based education group, or a control group receiving routine hospital education. Cardiovascular disease risk awareness and medication adherence will be assessed before and after the educational intervention and during follow-up.

入选标准

  • Aged 18-65 years.
  • Diagnosed with coronary artery disease.
  • Newly initiated on antiplatelet therapy.
  • Conscious and able to communicate.
  • Physically and cognitively able to use a virtual reality headset.
  • Able to read, write, and understand Turkish.
  • Willing to provide written informed consent.
  • Willing to participate in the education and follow-up process.
  • Has not previously received education about coronary artery disease.
  • Able to use a smartphone.

排除标准

  • Cognitive impairment or mental illness preventing communication.
  • Visual or hearing impairment preventing participation in the virtual reality education.
  • Participation in another clinical study at the same time.
  • Diagnosis of malignant disease.
  • Bleeding disorder or active bleeding risk.
  • Hospitalization within the previous 3 months due to acute myocardial infarction, stroke, or another serious cardiovascular event.
  • Unable to understand the educational materials.
  • Refusal to provide written informed consent.

时间窗: baseline and immediately after the educational intervention

Change in cardiovascular disease risk awareness, assessed using the Turkish validated 22-item Cardiovascular Disease Risk Awareness Assessment Scale (CVD-RAAS). The knowledge domain score ranges from 0 to 8, with higher scores indicating higher knowledge about myocardial infarction and stroke. The total score for the 14-item Likert-type awareness subscales ranges from 14 to 56, with higher scores indicating greater awareness of cardiovascular disease risks, healthier dietary intentions, and higher readiness for behavioral change.

medication adherence report scale score

时间窗: Baseline, immediately after the intervention, and 3 months follow-up

Change in medication adherence, assessed using the Turkish validated 5-item Medication Adherence Report Scale (MARS-5). Each item is scored on a 5-point Likert scale (1 = always to 5 = never). Total scores range from 5 to 25, with higher scores indicating greater medication adherence.

Cardiovascular Disease Risk Awareness Assessed by the Cardiovascular Disease Risk Awareness Assessment Questionnaire

时间窗: Baseline, immediately after the intervention, and 3 months after the intervention.

Cardiovascular disease risk awareness will be assessed using the Turkish version of the Cardiovascular Disease Risk Awareness Assessment Questionnaire (CDRAAQ), a 22-item instrument consisting of an 8-item knowledge section and three subscales: perceived risk of heart attack/stroke, healthy eating intentions, and perceived benefits and intentions to change. The knowledge items are scored as correct or incorrect, with each correct response receiving 1 point. The subscale items are rated on a 4-point Likert scale from 1 (strongly disagree) to 4 (strongly agree), with reverse scoring applied where appropriate. Higher scores indicate greater cardiovascular disease risk awareness, greater knowledge of heart attack and stroke, and greater readiness for health-related behavioral change.
