- •1. Patients with advanced cancer (Any cancer).
- •2. Newly referred to Palliative Medicine outpatient department
- •3. Patient over the age of 18 years and able to provide informed consent, or, through
- •surrogates, if the patient is sick and not being able to provide consent.
- •4. The geographical area in greater Mumbai where regular home care services are
- •not provided: Mira, Bhayandar, Uttan, Dongri, Kashimira, Kasarvadavali, and parts of
- •Thane (West).

- •1. Patients who do not agree with home-based care
- •2. Patients who do not have access to any local general practitioner
- •3.Patients for whom reliable contact number or method of communication are not
