The results showed that after the experiment, the experimental group had significantly higher mean scores in terms of knowledge, perceived benefits, perceived severity, self-efficacy, and behaviors to slow the progression of kidney disease than the control group . Further, the experimental group had significantly better clinical outcomes (SBP, DBP, eGFR, BUN, creatinine, albuminuria, FBS, and HbA1c) than the control group.

入选标准
- •1. patients with diabetes and hypertension with CKD 3 complication
- •2. aged 40 to70 years,
- •3. blood pressure > 140/90 mmHg at least once in the last six months
- •4. HbA1C > 7% in the last six months
- •5. able to communicate, read, and write in Thai
- •6. lived with their family
- •7. no complications or severe illnesses during the study that might have caused any obstacle to activity participation.
排除标准
- •1.Need to be relocated away from Samutprakarn province during the study.
- •2.Request to be withdrawn from the study.
