Ascorbic acid improves thrombotic function of platelet during orthotopic living donor liver transplantation via modulation of the function of E3 ubiquitin ligase Cbl-b and c-Cbl

- Thromboelastographic study The comparison between C and N group at the baseline measurement showed significant decrease in clotting time (CT) and clot formation time (CFT) but increases in alpha-angle, maximum clot firmness (MCF), the area under curve from the start of measurement to 6 minutes (AUC), and clot formation rate (CFR) in the N group compared to the C group, which coincides with the previous finding that hemodilution results in hypercoagulaton. However, the N group did not showed the significant differences in the amplitude 10 (A10) and 20 (A20) minutes after CFT, MCF-time (MCF-t), maximum lysis (ML) and lysis index at 60 minutes (LI60) compared to the C group. In the comparison between N and A group of baseline measurements, the A group showed significant increases of alpha-angle, amplitude 10 and 20 minutes after CFT (A10 and A20), MCF, MCF-time (MCF-t), AUC, and LI60 and significant decreases of CT, CFT, time to maximum velocity of clot formation (MAXV-t), and ML. In the analysis of thromboelastographic parameters measured at 120 minutes after reperfusion, there were significant increases in CFT, alpha-angle, A10 and 20, MCF, time to maximum velocity of clot formation (MAXV-t), AUC and decrease of MCF-t in the N group compared to the C group. These results may coincide with the findings from the comparison between the C and N group of baseline measurements, which did not result in the decrease of fibrinolysis. Compared to the C group, all parameters of the A group showed significant changes at the time of 120 minutes after reperfusion. However, MCF, ML, LI60 and MCF-t of the A group showed significant changes in comparison to the N group, which can be postulated as improvements of anti-fibrinolytic function in the A group. 2)Western blotting study Western blotting revealed that Cbl-b expressions of A group were significantly increased at the baseline and 120 minutes after reperfusion. However, we could not identify significant phosphorylations of Cbl-b in all three groups (Data not shown). These findings coincide with Daniel et al.’s previous study which could not observe significant phosphorylation of Cbl-b after stimulation by glycoprotein VI agonist in human platelet. However, expression of Cbl-b was most prominently increased in the A group baseline and 120 minutes after reperfusion. Phosphorylation of c-Cbl was decreased in the N and A group at both times we analyzed. However, the decrease was more significant in the A group.

- •Recipients who underwent elective living donor liver transplantation at our hospital, a university-affiliated hospital, were enrolled into this study.
排除标准
- •In order to minimize factors that can affect platelet function during liver transplantation, we excluded patients who had previously received liver transplantation because of the possibility of large amount of bleeding and transfusion, and patients who have been transfused any kind of blood component within 1 week before surgery. We also excluded patients who received platelet concentrates before blood sample was taken at the time of 120 minutes after reperfusion.
