- •Persons who are 20 years of age or older
- •Those who mainly take their own child to medical facility when he/she was sick.
- •Those who have taken their own child to a medical facility at least once within the past year.
- •Those whose child is 14 years old or younger when taken to a medical facility.

- •The individual and any family member with a health care provider
