- •Patients with Type 2 respiratory failure with arterial PCO2 of greater than and equal to 45mm hg.
- •Able to take own respiratory efforts

- •Denying consent
- •Anatomical lesions of upper airway and its obstruction
- •Central apnoea.
- •Blocked nasal passages
- •choanal atresia.
- •Trauma surgery to nasopharynx.
- •Pneumothorax
- •Requiring invasive ventilation.
- •Respiratory arrest
- •Unable to protect airways
- •Craniofacial trauma
