- •To analyze the health information management practices followed by Diabetes patients in management of their diabetes before and after the interventional mechanism
- •ï??Patients with in the age group of 40 to 65 who is diagnosed with Type 2 DM for a period of >= 2 years who are using glucometer
- •ï??HbA1c values between 7 to 10 %.
- •ï??Active patient at KH endocrinology OPD.
- •ï??Literate
- •ï??Patients using android smart phones.
