ADHD Linked to Nearly 50% Higher Odds of Gastrointestinal Symptoms in 1.9-Million-Person Review

People with attention-deficit/hyperactivity disorder face substantially higher odds of gastrointestinal problems than those without the condition, according to a University of Warwick analysis of more than 1.9 million people drawn from 23 studies. The review found an almost 50% increase in overall gastrointestinal symptoms among people with ADHD (搜索), with several individual conditions showing markedly stronger associations.
The findings, published in the Journal of Attention Disorders, represent the first systematic review devoted specifically to gastrointestinal symptoms in ADHD (搜索). The analysis pooled evidence from studies conducted in countries including the USA, Sweden, Germany and South Korea, covering participants aged 2 to 65.
Condition-Specific Risks
The strength of the association varied by symptom type. People with ADHD (搜索) had nearly double the odds of constipation (搜索), a 58% increase in irritable bowel syndrome (搜索), and more than four times the odds of encopresis (搜索), or loss of bowel control.
"Almost every gut symptom we looked at: constipation (搜索), IBS, even indigestion, appeared to be associated with ADHD (搜索)," said co-lead author Sarah Blake, a medical student at Warwick Medical School (搜索). "These symptoms can have a real impact on daily life, but they could easily be overlooked if the focus is on ADHD itself."
ADHD (搜索) is estimated to affect about 5% of children and 3-4% of adults worldwide, and reported diagnosis rates have risen sharply over the past four decades. Gastrointestinal problems are already well documented in autism spectrum disorder (搜索), and a growing body of evidence points to a "gut-brain axis" linking digestive health to neurodevelopmental conditions more broadly. Until now, however, the extent of gastrointestinal symptoms in people with ADHD had remained unclear.
Mechanistic Hypotheses Remain Unresolved
The researchers do not yet know why ADHD (搜索) and digestive problems appear to be connected, but they outline several possible contributors. ADHD-related traits such as impulsivity and irregular eating patterns may influence digestive health, and medications used to treat ADHD are known to produce gastrointestinal side effects, including constipation (搜索).
Another possibility involves the gut microbiome. Previous evidence has linked ADHD (搜索) with differences in gut bacteria, which may influence communication between the digestive system and the brain through the vagus nerve (搜索), described by the researchers as a major line of communication between gut and brain.
Implications for Clinical Practice
The authors say clinicians caring for people with ADHD (搜索) should be alert to gastrointestinal symptoms and consider asking about them during consultations. Patients with more complicated gastrointestinal symptoms may benefit from input from a dietitian or gastroenterologist.
The researchers emphasize that the findings identify an association rather than proving that ADHD (搜索) itself causes digestive problems. Further research is needed to determine how much of the connection involves underlying biological factors and how much may be related to medications or other influences.
"We can't yet say ADHD (搜索) causes these gut problems, or exactly what is driving the link, but the pattern is striking enough to warrant attention," said Dr. Saran Shantikumar, associate clinical professor at Warwick Medical School (搜索). "Clinicians caring for people with ADHD may want to consider asking about gastrointestinal symptoms, particularly if they may be affecting quality of life."
