Santhera Pharmaceuticals has announced positive topline results from a comprehensive five-year analysis of AGAMREE (vamorolone) in patients with Duchenne muscular dystrophy (搜索) (DMD (搜索)), demonstrating comparable long-term effectiveness to standard corticosteroids (搜索) while maintaining a significantly improved safety profile. The data, which includes first assessments from the ongoing GUARDIAN study, represents one of the most extensive long-term evaluations of the novel dissociative steroid in DMD patients.
Comprehensive Long-Term Analysis
The analysis encompassed up to 110 corticosteroid-naïve patients who initiated AGAMREE treatment between four and seven years of age in clinical studies and continued through various access programs, including the GUARDIAN study. Patients received AGAMREE for up to eight years, with a median follow-up of approximately five years. Most patients remained on higher doses of 4-6 mg/kg/day in real-world clinical settings throughout the observation period.
Maintained Efficacy with Superior Safety
Patients treated with AGAMREE maintained motor function over extended follow-up, demonstrating durable efficacy as measured by time to loss of ambulation comparable to standard of care corticosteroids (搜索) (p=0.91). In pre-specified subgroup analyses, no differences were observed compared with either daily deflazacort or prednisone, confirming the drug's therapeutic equivalence to established treatments.
The data revealed a significantly differentiated safety and tolerability profile versus traditional corticosteroids (搜索). Patients experienced a significantly lower rate of vertebral fractures (p=0.0061), a critical finding given the substantial impact of bone health complications in DMD (搜索) patients. Additionally, patients maintained normal growth without the stunting commonly seen with standard corticosteroids (p<0.0001), addressing one of the most concerning long-term effects of conventional steroid therapy.
Reduced Ocular and Metabolic Complications
The analysis showed fewer cases of cataracts than those typically observed in patients treated with glucocorticoids (搜索), including a notably lower incidence versus deflazacort (p<0.015). Importantly, no cases of glaucoma were observed throughout the study period. Changes in BMI or weight, when matched for height, did not differ on average, and no new safety signals were identified during the extended observation period.
Expert Perspectives on Clinical Impact
Prof Eugenio Mercuri, Professor of Pediatrics and Child Neuropsychiatry at Universita Cattolica del Sacro Cuore, commented: "These data provide important evidence that long term treatment with vamorolone provides durable efficacy, with a substantial reduction in the risk of spine fractures and of improvement in height, in contrast to what is observed with conventional steroids."
Prof Francesco Muntoni, Professor of Neurology at University College London, emphasized the growth benefits: "These preliminary data are encouraging. In particular, it is reassuring to see that children continue to grow in height without an apparent impact on treatment efficacy. Children treated with other corticosteroids (搜索) often experience significant growth stunting, which has a considerable impact on their quality of life."
Prof Craig McDonald, Professor of Physical Medicine & Rehabilitation and Pediatrics at UC Davis, highlighted the broader implications: "These data now clearly show the benefits of treating early and maintaining treatment at effective doses with vamorolone. Interestingly, the benefits we see in preservation of muscle function, height, and better bone health may also have important implications for older DMD (搜索) patients, for example, for upper-limb and respiratory function."
Novel Mechanism of Action
AGAMREE represents a novel approach to corticosteroid therapy, functioning as a dissociative anti-inflammatory drug that binds to the same receptor as glucocorticoids (搜索) but modifies downstream activity. Crucially, it is not a substrate for 11-β-hydroxysteroid dehydrogenase (搜索) (11β-HSD) enzymes that may be responsible for local drug amplification and corticosteroid-associated toxicity in tissues. This mechanism enables the drug to maintain therapeutic efficacy while reducing traditional steroid-related adverse effects.
Future Data and Clinical Development
Detailed results from this analysis are being submitted for presentation at a major international scientific conference in Q1 2026. The GUARDIAN study will continue collecting efficacy and safety data across a broader range of outcomes over the next three years, including assessments of upper limb function, pubertal development, eye health, and cardiac function.
