AHA Scientific Statement: Up to 400 mg of Caffeine Daily Is Safe for Most Adults and May Offer Cardiovascular Benefits

Consuming up to 400 mg of caffeine per day—equivalent to 3 to 5 cups of caffeinated coffee—is generally safe for most adults and does not increase cardiovascular risk, according to a new AHA scientific statement.

A new scientific statement from the American Heart Association (AHA), published in Circulation, concludes that for most adults, consuming up to 400 mg of caffeine per day—roughly equivalent to three to five 8-ounce cups of caffeinated coffee—is safe and does not increase cardiovascular risk. The statement, titled "Caffeine and Cardiovascular Disease (搜索)," represents the most comprehensive review of recent evidence on caffeine and heart health to date.
"Caffeine consumed in coffee is a key part of daily life for millions of people, and in our review of the most recent research, for most adults, intake of up to 400 mg of caffeine/day, the equivalent of up to 5 cups of caffeinated coffee per day without added sugars or fillers, is safe and does not increase cardiovascular risk," said Gregory M. Marcus, M.D., M.A.S., FAHA, chair of the scientific statement writing group and professor of medicine at the University of California, San Francisco School of Medicine. "However, high doses of caffeine, such as those found in energy drinks including energy shots, may have harmful effects on the heart and should be avoided."
Cardiovascular Benefits Observed in Observational Studies
The writing group reviewed evidence spanning prospective cohort studies, systematic reviews, meta-analyses, Mendelian randomization analyses, and randomized clinical trials. Across 30 prospective cohort studies, consumption of up to six cups of coffee daily was not associated with a higher risk of coronary artery disease (搜索). Light consumption (median 1.5 cups daily) and moderate consumption (median 3.5 cups daily) were each associated with an approximately 10% lower risk.
A dose-response meta-analysis of 20 cohort studies found a U-shaped association between coffee consumption and stroke (搜索), with the lowest risk at three to four cups daily—associated with a 21% lower risk compared with little or no consumption. Heart failure (搜索) studies similarly suggested a nonlinear association, with the lowest risk at approximately four cups daily.
Habitual coffee consumption was also consistently associated with a lower incidence of Type 2 diabetes (搜索) in observational studies. Similar associations observed with both caffeinated and decaffeinated coffee suggest that noncaffeine compounds may contribute to these benefits.
The statement authors caution that most favorable findings come from observational research, meaning residual confounding and healthy-user bias may have contributed to the reported associations. "It's also difficult to separate the effects of caffeine from other ingredients usually added to coffee, such as milk, cream, flavored syrups and/or sugar," the authors note.
Atrial Fibrillation (搜索) and Arrhythmia Findings
The statement provides specific guidance for patients with atrial fibrillation (搜索) (AF). Observational studies generally found no higher risk of AF with moderate coffee consumption, and some analyses found a lower risk. In the DECAF randomized clinical trial, patients with AF who consumed at least one cup of caffeinated coffee daily had a 39% reduction in recurrent AF risk compared with those who avoided coffee.
This finding aligns with a Class 3 recommendation in current AF guidelines advising clinicians not to recommend caffeine avoidance solely to reduce AF risk. "Physicians generally do not need to advise patients with atrial fibrillation (搜索) to avoid moderate coffee consumption solely to reduce recurrence risk," the writing group concluded.
However, the effect did not extend uniformly to other rhythm abnormalities. In the CRAVE randomized case-crossover trial involving 100 ambulatory adults, caffeinated coffee increased premature ventricular contractions (PVCs) but did not increase premature atrial contractions.
Energy Drinks and High-Dose Caffeine Pose Risks
The statement draws a sharp distinction between moderate coffee consumption and high-dose caffeine from energy drinks and shots. Energy drink shots may contain 40–69 mg of caffeine per fluid ounce—three to four times more than regular caffeinated coffee. These products have been associated with increased risk of high blood pressure and irregular heart rhythm, even in healthy adults younger than age 30.
"Potential health benefits that have been observed among individuals consuming caffeine that occurs in natural substances should not be extrapolated to synthetic products or high-dose caffeine. Indeed, although data on energy drinks are limited, available studies generally suggest cardiovascular harm," Marcus and colleagues wrote.
Case reports and small series have described AF following energy-drink consumption among otherwise low-risk patients. Extremely high doses of caffeine, particularly in concentrated powder or capsule form, have been associated with ventricular fibrillation and sudden death.
Coffee Preparation and Additives Matter
The statement highlights that coffee preparation methods influence cardiovascular effects. Cafestol, a compound present in unfiltered coffee—including espresso, French press, Turkish coffee, and boiled coffee—was associated with higher levels of low-density lipoprotein cholesterol in randomized trials. Paper-filtered and instant coffee do not contain cafestol.
Adding sugar, flavored syrups, milk, or cream to coffee likely reduces its potential health benefits, and more research is needed to understand the impact of these additives.
Individualized Approach Recommended
The writing group emphasized that there is no "one-size-fits-all" strategy for caffeine consumption. Genetics, metabolism, age, medications, underlying health conditions, and past caffeine use all influence individual responses. Short-term effects of caffeine may include temporary increases in blood pressure, heart rate, blood sugar, and alertness, and some people may experience heart palpitations or sleep disruption.
"Although research suggests that caffeine consumption may be associated with certain cardiovascular benefits for some people, it's important to remember that there is no 'one-size-fits-all' strategy for safe caffeine consumption," Marcus said. "What may be a reasonable amount for one person could cause unwanted effects, such as heart palpitations, anxiety or sleep disruption, in another. That's why it's important to pay attention to how your body responds to caffeine and talk with your healthcare team about what is right for you."
The statement was prepared by the volunteer writing group on behalf of the AHA's Council on Lifestyle and Cardiometabolic Health; the Council on Clinical Cardiology; and the Stroke (搜索) Council. Due to the lack of sufficient, rigorous data to develop guidance on caffeine intake recommendations, caffeine was not referenced in the Association's 2026 Dietary Guidance to Improve Cardiovascular Health.
