Avelumab Shows Modest Activity in Advanced Penile Cancer but Fails to Deliver Transformative Survival Benefits

The PD-L1 (搜索) inhibitor avelumab demonstrated modest but durable activity in a small subset of patients with advanced penile cancer (搜索), according to results from the phase 2 ALPACA trial presented at the 2025 European Society for Medical Oncology Congress in Berlin, Germany. While the study met its primary endpoint, the limited survival benefits underscore the continued challenges in treating this rare and aggressive malignancy.
Study Results and Clinical Outcomes
The multicenter, single-arm trial enrolled 25 patients with locally advanced inoperable or metastatic penile cancer (搜索) who were refractory to or unfit for platinum-based chemotherapy. Of the 23 evaluable patients, avelumab achieved an investigator-assessed objective response rate of 17% (4 patients), consisting entirely of partial responses with no complete responses observed.

Despite the durable responses, survival outcomes remained disappointing. At a median follow-up of 15 months, median progression-free survival was 1.7 months (95% CI: 1.5-1.8), and median overall survival was 3.9 months (95% CI: 2.6-9.9). The median number of treatment cycles completed was 3, with treatment ongoing in only 3 (13%) patients at the time of analysis.
Patient Population and Treatment Context
The study population had a median age of 58 years (range, 41-72), with 30% having an ECOG performance status of 2. Notably, 83% had visceral metastases affecting lung, liver, bone, or soft tissue, and 96% had received one prior line of platinum-based chemotherapy. Six patients were HPV (搜索)-positive, 5 were HPV-negative, and 12 had unknown HPV status.
Patients received avelumab 10 mg/kg intravenously every 2 weeks until disease progression or unacceptable toxicity. The study was conducted under a Simon 2-stage design targeting an objective response rate greater than 12.5%.
Safety Profile and Tolerability
Avelumab demonstrated a manageable safety profile consistent with its known characteristics, with no new safety signals or treatment-related deaths reported. The most common adverse events included infusion-related reactions (grade 1/2, 22%; grade 3+, 4%), edema in limbs (13%), fatigue (9%), and nausea (13%).
Immune-related adverse events occurred in several patients, including myalgia/arthalgia/arthritis (grade 1/2, 13%; grade 3+, 4%), maculopapular rash (9%), and various laboratory abnormalities. Of patients who discontinued treatment, 83% stopped due to disease progression or death, while only 4% discontinued due to adverse events.
Biomarker Analysis and Future Directions
Preliminary correlative studies failed to establish a clear correlation between HPV (搜索) status and patient outcomes, though this analysis was limited by the high number of patients with unknown HPV status. The researchers noted that a high neutrophil-to-lymphocyte ratio may predict worse outcomes, with additional correlative studies ongoing.
"In conclusion, in patients with advanced penile cancer (搜索) refractory to platinum-based chemotherapy, avelumab was well tolerated and achieved an objective response rate of 17%, with a median duration of response of about 15 months, suggesting a small subset of patients may benefit. But biomarkers are urgently needed to understand who these patients are and to refine patient selection," Sridhar explained.
Clinical Implications and Unmet Need
The study underscores the significant treatment challenges in penile cancer (搜索), a rare malignancy with approximately 38,000 new cases diagnosed worldwide annually. Up to 50% of penile cancer cases are HPV (搜索)-related, and approximately 30% to 60% express PD-L1 (搜索), providing a rationale for immune checkpoint inhibitor therapy.
Currently, no second-line treatment options exist for patients with advanced penile cancer (搜索), highlighting a significant unmet medical need. Standard first-line platinum-based chemotherapy offers limited efficacy with an objective response rate of approximately 30% and median progression-free survival of only 3 months.
"However, PFS and OS were very short, suggesting this data is not transformative. Future studies will need to consider if these agents should be evaluated in earlier disease settings, or in combination with novel therapeutic strategies," Sridhar noted.
The researchers emphasized that large-scale collaborations, innovative study designs, and increased funding will be essential to accelerate drug development and improve outcomes for patients with rare tumors, including penile cancer (搜索).
