Pre-TKI Era: Limited Options and Poor Outcomes
Before the development of tyrosine kinase (搜索) inhibitors (TKIs), CML (搜索) treatment consisted of only two options, neither of which provided satisfactory results. "In the standard of care prior to TKI development, there were 2 options: 1 was interferon, usually combined with low doses of cytarabine, and the other was transplant. Neither one was great," Cortes explained.
Interferon therapy proved particularly challenging due to its significant toxicity profile and limited efficacy. Complete cytogenetic responses, considered the benchmark for meaningful treatment response, occurred in only 20% to 25% of patients, accompanied by substantial adverse effects that made the treatment difficult to tolerate.
Transplantation, while potentially curative, faced numerous limitations that restricted its applicability. Age restrictions, donor availability, toxicity concerns, and mortality rates made this option unsuitable for many patients. "In those days, we were facing difficult choices for patients, and many did not have great options. The outcomes were not good, overall," Cortes noted.
The Imatinib Revolution
The approval of imatinib (Gleevec) in 2000 marked a pivotal moment in CML (搜索) treatment history. The drug's impact was evident from early clinical trials, with responses observed even in heavily pretreated patients who had failed previous therapies including transplantation and interferon.

"Patients would cry if they were randomized to interferon-alfa," Cortes recalled, highlighting the stark difference in treatment experience between the two approaches.

This agent offers activity in settings similar to ponatinib but through a completely different mechanism, potentially providing improved tolerability profiles.
According to Cortes, asciminib currently offers the best opportunity for patients seeking treatment-free remission due to its ability to achieve higher rates of deeper responses early in treatment, combined with superior tolerability compared to other available options.
Multidisciplinary Care Evolution
The transformation of CML (搜索) from a specialized disease treated only in academic centers to a manageable chronic condition has necessitated a multidisciplinary approach to patient care. The extended survival of CML patients has created new challenges requiring expertise from various healthcare professionals.
"Because these patients live longer lives, we must focus on other elements, like drug-drug interactions—our patients end up taking other drugs for other conditions," Cortes explained. This evolution has integrated pharmacists for drug interaction management, nurse practitioners for ongoing patient support, pathologists for advanced monitoring techniques, and cardio-oncologists for cardiovascular risk management.
Treatment Selection Considerations
Current treatment selection involves comprehensive patient evaluation beyond traditional medical factors. Cortes emphasizes the importance of understanding patient goals, including preferences for longevity, treatment-free remission potential, adverse effect tolerance, and financial considerations.
For patients prioritizing treatment-free remission and quality of life, asciminib represents the preferred option due to its superior response rates and tolerability profile. Patients with high cardiovascular risk may benefit from imatinib, which carries the lowest risk of arterial occlusive events. Financial considerations may also influence treatment selection, with generic imatinib offering a cost-effective option while maintaining therapeutic efficacy.
Future Directions and Cure Potential
Two primary research objectives define the future of CML (搜索) treatment. First, expanding the proportion of patients who can successfully discontinue therapy beyond the current 25% to 30% rate. Second, addressing the subset of patients with additional genetic abnormalities, comprising approximately 20% of cases, who demonstrate inferior treatment responses.
Regarding the potential for cure, Cortes believes the field is progressing toward this goal. "We can stop therapy effectively in some patients, which is equivalent to a cure. If [a patient has] done well, and you can stop therapy and the disease doesn't come back, that's essentially what we think about as a cure," he stated.
