Coffee Consumption Linked to Significant Reductions in Cirrhosis, Liver Cancer, and Liver-Related Death in Large UK Biobank Study

A large prospective analysis from the UK Biobank has delivered some of the strongest observational evidence to date linking regular coffee consumption to substantially better long-term liver health. The study, published in Clinical Gastroenterology and Hepatology, found that individuals who drank five or more cups of coffee per day had a 32% lower risk of developing cirrhosis (搜索), a 47% lower risk of liver cancer (搜索), and a 42% lower risk of liver-related death compared with non-coffee drinkers over a median follow-up of approximately 13 years.
"What surprised me most was how consistent the signal was," said lead study author Hyunseok Kim, MD, PhD, MPH, a transplant hepatologist and assistant professor of medicine at Cedars-Sinai Medical Center in Los Angeles. "The practical message is that, for people who already enjoy coffee and tolerate it well, moderate coffee intake can be part of a liver-healthy lifestyle."
Study Design and Population
Researchers analyzed health data from 354,957 UK Biobank participants who enrolled between 2006 and 2010. All participants were between 40 and 69 years old at baseline, and none had cirrhosis (搜索) or liver cancer (搜索) at the time of enrollment. Participants self-reported their coffee consumption—including whether they drank regular or decaffeinated coffee and whether they added sweeteners—via a questionnaire.
The study went beyond prior epidemiological investigations by incorporating liver MRI scans for nearly 30,000 participants and blood protein analyses in another 44,000 individuals. "This allowed us to look not only at whether coffee was associated with better outcomes, but also at potential biological pathways behind that association," Kim explained.
Dose-Response Relationship and Clinical Outcomes
The liver health benefit appeared to follow a dose-response pattern: as coffee intake increased, liver disease risk progressively decreased. While the most pronounced risk reductions were observed in the highest consumption group (five or more cups daily), participants who drank one to two cups and three to four cups per day also experienced meaningful reductions in liver disease risks.
"These findings reinforce the growing body of evidence that coffee consumption is associated with substantially better long-term liver health," Kim said. "Importantly, we observed a dose-response relationship, with greater coffee intake associated with progressively lower risks of cirrhosis (搜索), liver cancer (搜索), and liver-related mortality."
Imaging and Proteomic Evidence
MRI scans revealed that people who drank more coffee had lower levels of liver fat and liver iron, as well as lower odds of elevated cT1—an MRI marker linked to liver inflammation and fibroinflammation. Blood proteomic analyses further showed that coffee drinkers had favorable changes in circulating proteins involved in fibrosis, inflammation, and liver function.
"This is one of the most novel aspects of our study," Kim noted. "Rather than relying solely on clinical outcomes, we found that coffee consumption was also associated with healthier liver tissue characteristics on MRI, including lower liver fat and fibroinflammation, as well as favorable changes in circulating proteins involved in fibrosis, inflammation, and liver function. These independent lines of evidence converge to support the biological plausibility of coffee's beneficial association with liver health."
Mechanisms: Beyond Caffeine
Both caffeinated and decaffeinated coffee drinkers experienced protective benefits, indicating that caffeine alone does not explain the observed associations. Kim pointed to the hundreds of bioactive compounds in coffee—including chlorogenic acids (搜索), polyphenols (搜索), and diterpenes—as likely contributors.
Pichamol Jirapinyo, MD, MPH, an assistant professor of medicine at Harvard Medical School and a spokesperson for the American Gastroenterological Association who was not involved in the research, emphasized the role of antioxidants and polyphenols (搜索). These compounds may reduce oxidative stress and chronic inflammation, both of which contribute to liver injury. Experimental studies suggest they may also decrease fat accumulation in the liver, reduce the activation of scar-forming cells, and limit the development of fibrosis.
Meridan Zerner, MS, RDN, CSSD, LD, CHWC, founder of Meridan Zerner Nutrition in Dallas, who was also not involved in the study, added that coffee's components "have been shown to improve insulin sensitivity and support healthier blood sugar regulation. Regular coffee consumption has also been associated with lower liver fat and slower progression of liver fibrosis, both of which are important because fatty liver disease and cirrhosis (搜索) are major risk factors for liver cancer (搜索)."
Limitations and Cautions
Despite the robust findings, the study has important limitations. It is observational in nature, meaning it can demonstrate association but not causation. "Coffee intake was self-reported, and people who drink coffee may differ from nondrinkers in other ways despite careful adjustment," Kim acknowledged. The researchers adjusted for factors including age, sex, smoking, alcohol use, body mass index, diabetes, high blood pressure, and genetic variants linked to liver disease risk, but residual confounding remains possible. Additionally, the UK Biobank population is largely white, which may limit the generalizability of the findings.
"Overall, I think this is one of the strongest observational studies we've seen on coffee and liver health," said Jirapinyo. "What makes it stand out is not just its size, but also the integration of clinical outcomes," including MRI results and blood work.
Clinical Guidance
Kim emphasized that the findings should not prompt non-coffee drinkers to start the habit solely for liver protection. "Coffee should complement, not replace, the fundamentals—avoiding excessive alcohol, maintaining a healthy weight, exercising, and managing diabetes, cholesterol, and blood pressure," he said.
For most adults who already tolerate coffee well, one to four cups per day appears to be a sensible range based on this study and broader evidence, Jirapinyo noted. "More isn't necessarily better. This study observed an association, not proof of causation, and it wasn't designed to determine the 'optimal' dose of coffee."
People who are pregnant, have severe reflux, heart arrhythmias, anxiety, insomnia, or caffeine sensitivity should be cautious about regular coffee intake. While the protective findings also applied to those who added sugar or artificial sweeteners, additives were linked to "modestly higher" MRI markers for liver inflammation. "I would generally favor unsweetened coffee or minimizing sugar, especially for people with obesity, diabetes, or metabolic risk factors," Kim advised.
