GLP-1 Receptor Agonists Reduce Amputation Risk and Cardiovascular Events in High-Risk Diabetes Patients

Patients with diabetes (搜索) who have experienced major limb complications face substantially reduced risks of amputation (搜索) and cardiovascular events (搜索) when treated with GLP-1 receptor agonists compared to DPP-IV inhibitors (搜索), according to new research analyzing over 17,000 patients in Taiwan's national healthcare database.
The retrospective cohort study, published in JAMA Network Open, examined outcomes in patients with diabetes (搜索) and a history of major adverse limb events (搜索) (MALEs) - a population at extreme risk for future complications. Over a mean follow-up of 4.1 years, GLP-1 receptor agonists demonstrated consistent protective benefits across multiple organ systems.
Significant Reduction in Limb Complications
Among the 17,288 patients analyzed (mean age 70.7 years, 57.9% men), those treated with GLP-1 receptor agonists showed a 10% reduction in risk for lower limb revascularization and nontraumatic major and minor amputation (搜索) compared to DPP-IV inhibitors (搜索) (subdistribution HR = 0.9; 95% CI, 0.83-0.97). This benefit was primarily driven by a 14% reduction in amputation risk (subdistribution HR = 0.86; 95% CI, 0.75-0.98).
"The amputation (搜索) finding suggests that GLP-1s may play a particularly vital role in preventing the most severe outcomes of advanced limb disease," said Pao-Hsien Chu, MD, professor and physician in the department of cardiology at Chang Gung Memorial Hospital (搜索) in Taipei, Taiwan, who led the research.
Cardiovascular and Mortality Benefits
The protective effects extended well beyond limb preservation. Patients receiving GLP-1 receptor agonists experienced a 38% reduction in major adverse cardiovascular events (搜索), defined as cardiovascular death, ischemic stroke (搜索), and myocardial infarction (搜索) (HR = 0.62; 95% CI, 0.58-0.65). Cardiovascular death specifically was reduced by 43% (HR = 0.57; 95% CI, 0.53-0.61).
All-cause mortality showed a 37% reduction in the GLP-1 group (HR = 0.63; 95% CI, 0.6-0.66), while progression to dialysis was reduced by 39% (subdistribution HR = 0.61; 95% CI, 0.54-0.7) compared to DPP-IV inhibitor treatment.
Addressing a Critical Evidence Gap
The study addressed a significant gap in clinical evidence for this high-risk population. "Patients with diabetes (搜索) and a history of major adverse limb events (搜索) face a significantly higher risk of further cardiovascular and limb complications," Chu explained. "While current guidelines recommend GLP-1 receptor agonists to reduce cardiovascular risks, there has been limited evidence regarding their effectiveness for secondary prevention, specifically in this high-risk population with prior MALEs."
Previous studies had primarily focused on patients with low baseline risk for limb events, leaving clinicians with limited guidance for treating those who had already experienced major limb complications.
Multi-Organ Protective Mechanisms
The researchers attributed the broad protective benefits to the multi-organ effects of GLP-1 receptor agonists. "Unlike DPP-IV inhibitors (搜索), which have relatively neutral cardiovascular effects, GLP-1s have shown significant associations with reduced risks of MACEs, cardiovascular death, and progression to long-term dialysis," Chu noted.
The study analyzed data from Taiwan's National Health Insurance Research Database from October 2012 to December 2023. Of the total cohort, 1,583 patients initiated GLP-1 receptor (搜索) agonist therapy while the remainder received DPP-IV inhibitors (搜索). Major adverse limb events (搜索) were defined as chronic limb-threatening ischemia (搜索), lower limb revascularization, or nontraumatic minor and major amputation (搜索).
Clinical Practice Implications
The findings support preferential use of GLP-1 receptor agonists for secondary prevention in patients with diabetes (搜索) who have already experienced major limb events. "This population is at extreme risk, and our data suggest that GLP-1s offer critical protective benefits for limb, heart and kidney health that may not be equally provided by other glucose-lowering therapies like DPP-IV inhibitors (搜索)," Chu emphasized.
The consistent reductions across limb, cardiovascular, renal, and mortality outcomes suggest these medications may play a central role in comprehensive risk reduction strategies for this vulnerable patient population.
