The findings, presented at the American Heart Association Scientific Sessions 2025 in New Orleans, could transform prescribing practices for these widely-used weight loss medications.
Study Challenges Clinical Hesitation
Since their initial approval in 2005, GLP1RAs (搜索) have become recognized for their efficacy in managing type 2 diabetes (搜索) and promoting weight loss. However, clinicians have often exercised caution when prescribing these agents to individuals with severe hypertriglyceridemia (搜索), defined as triglyceride levels exceeding 500 mg/dL. This hesitation stems from the intrinsic risk of pancreatitis (搜索) associated with elevated triglycerides, combined with the pancreas (搜索)-targeted mechanism of GLP1RAs.
The retrospective observational study analyzed electronic health records of 346,667 patients treated at Intermountain Health (搜索) from January 2006 to April 2025, focusing on adults over 18 years diagnosed with type 2 diabetes (搜索) or having a body mass index above 27. Within this large cohort, 3,834 patients (1.1%) were prescribed GLP1RAs (搜索).
Protective Effects Observed
The study found no increased incidence of pancreatitis (搜索) among patients on GLP1RA therapy, regardless of their triglyceride status. More significantly, for patients with elevated triglycerides who had no prior history of pancreatitis, treatment with GLP1RAs (搜索) was associated with a four-fold reduction in the risk of developing pancreatitis compared to patients not receiving these medications.
"Pancreatitis (搜索) is incredibly painful and can be deadly. Once you have a patient with acute pancreatitis you want no part of causing it again," said Leslie Iverson, PA-C, cardiovascular prevention and research clinician at Intermountain Health (搜索). "But these findings show no link between pancreatitis and patients with high triglycerides taking a weight loss drug. Even better, we found that these drugs may offer protection against ever having it in the first place."
Mechanisms Behind Protection
The pathophysiological mechanisms by which GLP1RAs (搜索) may reduce pancreatitis (搜索) risk are under investigation. GLP1RAs improve glycemic control and promote weight loss while also modulating lipid metabolism, which can positively impact triglyceride levels. "Our findings show that HTG is not a reason to withhold this class of medication from appropriate patients, if they would benefit otherwise," Iverson explained. "This is an important finding that helps enhance our treatment options."

The retrospective approach strengthens the validity of conclusions by providing real-world evidence from a large patient population.
