Rachael Johnson, 41, from Plymouth, received an intrathecal morphine pump at Derriford Hospital that has effectively eliminated her severe cancer-related pain and allowed her to continue with cancer treatment.
Johnson was diagnosed with bowel cancer (搜索) in 2023 and faced a grim prognosis. "I was basically stage four [cancer] and they were saying it wasn't curable, only treatable... and then, all of a sudden... I am not on palliative care any more," she said. Three years after diagnosis, she described living a "completely normal" life, adding: "I can do just about everything, really, and I am not in any pain at all."
How the Intrathecal Pump Works
The intrathecal pump is a device surgically implanted beneath the abdomen that delivers medication through a catheter directly into the intrathecal space of the spine. This targeted delivery mechanism represents a fundamental departure from conventional systemic opioid administration.
Nurse consultant Lindsey Pollard, who works with the Chronic Pain Management Team at University Hospitals Plymouth NHS Trust, explained the pharmacological advantage: "This pump allows us to infuse medication into a targeted area within the body so we can get very good pain relief for the patient. The morphine in her is tiny amounts, but, because it doesn't go into the bloodstream or through the brain, the side effects are completely eradicated."
Johnson is one of 25 patients who have received an intrathecal pump through the Plymouth service. The team is among only a few in the UK to offer this intervention.
A Family's Perspective
Johnson's mother, Sue Johnson, who served as her caregiver, described the transformative impact. "It's revolutionized Rachael's life because the amount of morphine I had to administer to Rachael before she had access to the pain pump was too much. I knew her body was never going to cope with it all."
Sue noted that her daughter's body was deteriorating under the previous pain management regimen, but the pump enabled her to eat properly and tolerate cancer treatment. "I am shocked that a pain pump and pain management can make so much difference to keeping someone alive. Without that, you get eaten up by the pain," she said.
Broader Clinical Impact
Pollard reported that the life-changing results observed in Johnson have been replicated across the patient cohort. "It's just amazing to see patients like Rachael who've been unwell going off on holiday and enjoying their life. We've got patients who are back on horses, back on tractors. They are out living their lives."
The clinic serves patients who often live with persistent pain that has continued beyond the point of natural healing, with some having been in pain for many years. The service offers a multidisciplinary range of treatments including injections, medication, physiotherapy, clinical psychology, and implants.
Cautious Optimism and Service Expansion
Anaesthetist Dr David Hutchins, a consultant for the Chronic Pain Management Team, tempered expectations while acknowledging the treatment's profound impact. "Rachael is one wonderful example of a cohort of patients that me and other centres around the world are seeing," he said. "It needs to be caveated. This is a specific management for very few patients, unfortunately, but it changes people's lives dramatically."
In March, the service relocated to a purpose-designed space at Plymouth Marjon University as part of a collaboration between the university and the trust. The additional space is expected to allow the team to develop new programmes focused on helping patients manage pain and improve their overall wellbeing. Johnson and her mother participated in the new unit's official opening ceremony.
Johnson expressed hope that other patients would benefit similarly, noting: "Usually people have it to get them through the last part of their life, but, for me, it's been totally different." She added that if she remains cancer-free until 2029, the pump can be removed.
