The Molecular International Prognostic Scoring System (IPSS-M (搜索)) is reshaping how clinicians identify patients with myelodysplastic syndromes (搜索) (MDS) who harbor hidden high-risk biology despite appearing to have clinically lower-risk disease. By incorporating molecular mutations into prognostic assessment, the tool formalizes a practice that experienced hematologists had long employed anecdotally—flagging patients for earlier allogeneic transplant evaluation before they cycle through all standard lower-risk treatments.
Amer Zeidan, MBBS, MD, professor of medicine at Yale School of Medicine and chief of the Division of Hematologic Malignancies at Yale Comprehensive Cancer Center (搜索), explained that even before IPSS-M (搜索), clinicians recognized that certain patients classified as lower-risk by the IPSS or IPSS-R had worse outcomes due to adverse molecular mutations. "We would consider those patients for transplant relatively earlier in their disease course. We would not wait until they had gone through all the traditional treatments because we know their survival is somewhat limited, especially for younger patients or others who are candidates for allogeneic transplant," Zeidan said during a Frontline Forum discussion.
Formalizing Molecular Risk Assessment

Raji Shameem, MD, a hematologist and medical oncologist at the Orlando Health Cancer Institute, echoed this perspective, stating that she has incorporated IPSS-M (搜索) into her practice for patient stratification. "I don't perform transplant, but it helps me identify those who need a transplant evaluation," Shameem said. She estimated that approximately 30% of patients can be re-stratified to higher-risk categories, while some may be moved to lower risk, aiding in the selection of appropriate disease-modifying therapies.
Eliminating the Intermediate-Risk Ambiguity
Akriti Jain, MD, a hematologist and medical oncologist at Cleveland Clinic, highlighted another key advantage of the IPSS-M (搜索): the elimination of the intermediate-risk category that existed in the IPSS-R. "IPSS-M has been great because it helped divide patients into lower risk and higher risk, removing the intermediate risk in IPSS-R, which was always that tough patient to treat, because intermediate risk can sometimes fall in lower risk and other times in higher risk," Jain said. This binary stratification streamlines the diagnostic process and clarifies treatment selection, reducing clinical uncertainty for a patient population that previously occupied a therapeutic gray zone.
