The European Journal of Pediatrics (搜索) retracted the 77-child study last week after reviewers identified multiple statistical errors and data inconsistencies that could not be resolved. The journal stated it "confirmed several of the concerns raised with the data and statistical analysis and was unable to replicate the results reported in the article from the dataset provided."
Weak Evidence Base Further Eroded
The retracted study, published in September 2024, had claimed that 24 weeks of daily oral leucovorin treatment reduced autism (搜索) symptom severity compared to placebo. However, pediatricians Thomas Challman from Geisinger College of Health Sciences (搜索) and Scott Myers discovered that numbers in the data tables did not add up correctly when they examined the research closely.
"The retraction of this paper removes a significant portion of the already weak evidence supporting the value of folinic acid as a treatment for autism (搜索)," Challman wrote. "Until we have acceptable evidence of safety and effectiveness, folinic acid use as a treatment for autism is not appropriate outside of a well-designed clinical trial."
The retraction leaves researchers with only four other randomized clinical trials testing leucovorin in autism (搜索), all of which have faced criticism for poor methodology, small sample sizes, and inadequate blinding. Dorothy Bishop, emeritus professor of developmental neuropsychology at the University of Oxford, characterized the overall research quality as "uniformly poor."
FDA Approval Controversy
The retraction comes as the U.S. Food and Drug Administration considers expanding leucovorin approval for cerebral folate deficiency (搜索), a condition that some research suggests affects up to 40 percent of autistic people. The FDA's decision, announced last year, was based on evidence from 23 studies in children with the rare genetic disorder, but many researchers have expressed skepticism about the limited evidence base.
"Folate receptor (搜索) autoantibodies, similar to those found in people with cerebral folate deficiency (搜索), may be more prevalent in autism (搜索) and may play a significant role in the neuropathogenesis of the condition," says Shafali Jeste, chair of pediatrics at the University of California, Los Angeles. "However, neither of those statements has been actually proven, so we're already working under a premise that is not evidence based."
Pivotal Trial Results Expected in 2026
Against this backdrop of controversy and limited evidence, researchers are closely watching a new trial led by Richard Frye at Rossignol Medical Center in Arizona. The study is testing leucovorin in 80 autistic children between ages 2.5 and 5, representing a significant increase in sample size compared to previous research.
The trial design includes 12 weeks of treatment versus placebo for half the participants, followed by an additional 12 weeks where all children receive leucovorin to gather safety data. Researchers are primarily measuring changes in social communication through parent questionnaires, while also tracking irritability, hyperactivity, repetitive behaviors, sensory sensitivities, and restricted interests.
"There currently is no treatment for core symptoms of autism (搜索)," Frye explained. "All the medications we have are just kind of Band-Aids that treat symptoms. This could be a treatment that not only improves symptoms for these kiddos but actually treats some of the underlying mechanisms."
Safety Questions Remain
The trial is also addressing lingering safety concerns about leucovorin use in autism (搜索). While the drug is widely considered safe for its approved uses in treating vitamin B9 deficiencies and cancer medication side effects, little is known about its effects specifically in autistic children.
"The number of families that have participated in these [past] studies have not been adequate to do a full safety study," notes Alycia Halladay at the Autism Science Foundation (搜索) in New York.
Frye's team is conducting safety assessments every two weeks for the first 12 weeks, then every four weeks thereafter, while collecting blood samples to monitor for changes in blood clotting, immune response, and organ function.
Mechanism of Action Under Investigation
Beyond measuring clinical outcomes, the trial includes brain scans before and after treatment to better understand leucovorin's mechanism of action. "We don't know exactly what leucovorin is doing, but what we think is happening is that the brain is getting more connections," Frye said.
Previous small studies have shown some promise. A 2016 trial of 23 autistic children with language difficulties found that 65 percent showed clinically meaningful improvement in verbal communication after 12 weeks of leucovorin treatment, compared to about 25 percent of children receiving placebo.
Scientific Community Divided
Despite the upcoming trial, the scientific community remains divided on leucovorin's potential. Some researchers worry that even the 80-child study may be too small to draw definitive conclusions, particularly since it's being conducted at a single site.
"I think that this is a good move in the right direction, but I do think that we are going to need additional studies done by additional people at other sites," Halladay said.
Jeste expressed skepticism about the entire research direction, suggesting that interest in leucovorin stems more from publicity than scientific merit. "I think the reason this specific drug has been of such interest is because of the publicity around it, not because we as scientists believe that this is the drug that we should be testing."
The retracted study's authors, led by Prateek Kumar Panda from All India Institute of Medical Sciences, acknowledged "unintentional statistical analysis errors" and plan to submit a revised version addressing the identified concerns.
