Medically Supervised Ketogenic Diet Extends Survival by 34% in Metastatic Pancreatic Cancer Patients

The study enrolled 32 patients with metastatic pancreatic ductal adenocarcinoma (搜索) who received gemcitabine, nab-paclitaxel, and cisplatin with or without the ketogenic diet intervention.
A medically supervised ketogenic diet delivered alongside standard chemotherapy has demonstrated remarkable survival benefits for patients with metastatic pancreatic cancer (搜索), according to results from a randomized Phase II clinical trial published in Cancer. The study, conducted by Virta Health (搜索) in partnership with USC Norris Comprehensive Cancer Center (搜索), Mayo Clinic, and HonorHealth Research Institute (搜索), showed that patients receiving the nutritional intervention lived 34% longer than those on standard diet.
Trial Design and Patient Population
The multicenter trial enrolled 32 evaluable patients with stage IV metastatic pancreatic ductal adenocarcinoma (搜索), a disease where median life expectancy typically ranges from 6 to 12 months. All participants received a triplet chemotherapy regimen consisting of gemcitabine, nab-paclitaxel, and cisplatin, and were randomized in a 1:1 ratio to either receive Virta's medically supervised ketogenic diet (MSKD) or continue with a standard diet.
Eligibility criteria required patients to be 18 years or older with histologically or cytologically confirmed metastatic pancreatic cancer (搜索), no prior systemic therapy for metastatic disease, measurable disease per RECIST v1.1 criteria, a Karnofsky score of 70% or higher, and adequate organ function.
Survival Outcomes
The integration of the MSKD with chemotherapy resulted in a median overall survival of 13.7 months compared with 10.2 months for patients receiving chemotherapy with a standard diet (HR, 0.58; 95% CI, 0.25-1.37; P = .107). Progression-free survival also showed improvement, with a median of 8.5 months in the MSKD arm compared with 6.2 months in the control group (HR, 0.53; 95% CI, 0.21-1.37; P = .096).
"Pancreatic cancer (搜索) is among the deadliest cancers, and improving outcomes by any margin is rare," said Adam Wolfberg, MD, MPH, Chief Medical Officer of Virta. "Seeing a nutritional intervention—delivered remotely and safely—make this level of impact is extraordinary."
Response Rates and Metabolic Markers
The disease control rate at 9 weeks was 93.8% in the MSKD arm compared to 87.5% in the non-MSKD arm (P = 1.000). A best overall response of partial response was observed in 68.8% of the MSKD arm versus 31.2% of the non-MSKD arm (P = .110).
Using daily ketone tracking, nutritional ketosis (0.5-3.0 mM) was achieved by 15 of 16 patients in the MSKD arm at some point during the study. The mean beta-hydroxybutyrate (BHB) levels were 0.49 (95% CI, 0.36-0.63), and the median proportion of days in ketosis was 39.4% (range, 0%-95.8%).
Safety Profile
The addition of the MSKD did not significantly increase the toxicity profile of the triplet chemotherapy regimen. MSKD-related adverse events included fatigue (n = 3), constipation (n = 3), weight loss (n = 3), decreased appetite (n = 1), dehydration (n = 1), dizziness (n = 1), nausea (n = 1), and body ache (n = 1), all of which were grade 1 or 2. No patients discontinued the MSKD due to adverse events.
Chemotherapy-related adverse events and treatment-related deaths did not occur during the study. There were no significant differences between treatment arms for grade 3 or higher chemotherapy-related adverse events including anemia, diarrhea, or platelet count decrease (all P >.2).
Mechanism and Future Directions
"The mechanism of action is not fully understood, but this nutrition approach has shown in preclinical work to improve the effect of anti-cancer therapy, and that was shown in this clinical trial," said Erkut Borazanci, MD, director of the Oncology Research Division of HonorHealth Research Institute (搜索) and one of the study's lead authors.
The dietary intervention included total carbohydrates limited to 30 grams or less per day and daily protein intake of 1.5 grams per kilogram of reference body weight, with other aspects prescribed individually through the Virta Health (搜索) platform using individualized nutrition, biomarker tracking, AI tools, one-on-one coaching, and continuous medical supervision.
"These results further validate that Virta's approach isn't about weight loss or calorie counting—it's about addressing the root cause of metabolic dysfunction that drives disease," said Wolfberg. "That's why our method works not only to reverse obesity (搜索) and type 2 diabetes (搜索), but now shows promising synergistic effects alongside traditional cancer therapies."
The researchers hope to confirm their findings through a larger study, as this trial represents a rare breakthrough in a field where decades of drug development have yielded only incremental gains in pancreatic cancer (搜索) treatment.
