Methotrexate Shows Cardiovascular Benefits Beyond Arthritis Treatment in New Study

Methotrexate, a cornerstone treatment for rheumatoid arthritis (搜索), demonstrated significant cardiovascular benefits beyond its anti-inflammatory effects in a new controlled study. The research, published in Annals of Medicine, found that the drug substantially lowered systolic blood pressure compared to another disease-modifying antirheumatic drug (DMARD) in newly diagnosed patients.
Study Design and Key Findings
The six-month controlled comparative study followed 62 treatment-naïve adults with newly diagnosed rheumatoid arthritis (搜索). Half received subcutaneous methotrexate while the other half took oral sulfasalazine. Researchers measured changes in blood pressure, arterial stiffness, and disease activity throughout the study period.
By six months, patients treated with methotrexate showed a mean reduction in systolic blood pressure of 7.4 mm Hg compared with those taking sulfasalazine (95% CI, −14.0 to −0.8; P = .03). Lead author Arduino A. Mangoni, MD, PhD, strategic professor and head of clinical pharmacology at Flinders University, described this reduction as clinically meaningful.
"We've known for a while that methotrexate helps with inflammation, but now we're seeing that it may also help lower blood pressure, which is a major risk factor for heart disease," Mangoni said. "This could be a big step forward in how we care for people with rheumatoid arthritis (搜索)."
Cardiovascular Risk Context in RA
Rheumatoid arthritis (搜索) affects approximately 1 in 100 people worldwide and is associated with systemic inflammation, endothelial dysfunction, and heightened cardiovascular disease (搜索) risk. Hypertension (搜索) and arterial stiffness are common comorbidities that increase the likelihood of heart attacks and strokes among individuals with RA.
While past observational studies have linked methotrexate to reductions in cardiovascular-related mortality and morbidity in autoimmune disease populations, those studies did not establish causality. The current findings provide prospective comparative evidence that methotrexate lowers systolic blood pressure in patients with RA, independent of improvements in joint disease activity or arterial stiffness.
Mechanisms Beyond Joint Inflammation
The blood pressure reduction appeared unrelated to changes in arthritis symptoms or arterial stiffness, suggesting methotrexate may help cardiovascular health through alternative mechanisms such as calming inflammation or improving blood vessel function. According to the researchers, this proposition is further supported by the lack of between-group differences in the use of other drugs affecting blood pressure, including antihypertensive medications, corticosteroids, and nonsteroidal anti-inflammatory drugs.
Genetic Factors Influence Response
The study also examined whether genetic variations influenced blood pressure outcomes. Researchers found that patients with certain single nucleotide polymorphisms (SNPs), including rs1801133 in the MTHFR (搜索) gene and rs2231142 in the ABCG2 (搜索) gene, experienced greater reductions in blood pressure while on methotrexate.
"The specific associations between rs1801133 (MTHFR (搜索)) and rs2231142 (ABCG2 (搜索)) and blood pressure changes during methotrexate treatment are supported by the concomitant lack of associations between these SNPs and blood pressure changes with sulfasalazine," the authors noted.
These findings suggest a potential role for pharmacogenetics in tailoring RA treatment, opening the door to more personalized treatment options where doctors could use genetic testing to predict who might benefit most from methotrexate's heart-protective effects.
Study Limitations and Future Directions
The researchers acknowledged several limitations, including that treatment allocation was not randomized but based on clinical judgment, raising the potential for confounding by indication. Additionally, the sample size was relatively small, and patient attrition was higher in the sulfasalazine group.
Study coordinator Sara Tommasi, PhD, medical scientist at Southern Adelaide Local Health Network (搜索), emphasized that while more research is needed to confirm these findings and understand exactly how methotrexate lowers blood pressure, the results are promising. "The results suggest that this well-known arthritis drug could also play a role in protecting heart health, especially in people who are at higher risk due to inflammation," she said.
