"Naegleria fowleri (搜索) is a microscopic amoeba. It's a free-living organism that lives in fresh water such as lakes and rivers. It can also live in poorly kept pools and water systems," said Lyssette Cardona, MD, an infectious disease specialist with Cleveland Clinic Florida.
How Infection Occurs
People become infected when water containing the amoeba enters the nose. From there, the amoeba travels along the olfactory nerve to the brain, where it directly consumes brain cells. Dr. Cardona explained that the brain-eating amoeba thrives in warm fresh water, which is why cases tend to make headlines over the summer.
The amoeba can also lurk in untreated or poorly chlorinated swimming pools, residential water heaters, and industrial warm-water discharge areas. It multiplies in water temperatures up to 46 degrees Celsius, making infections more likely during summer months and raising concerns about climate change. As the world warms, Naegleria fowleri (搜索) may be expanding its range, making infections more likely.
Importantly, the disease cannot be transmitted from person to person, nor can it be contracted from salt water or from swallowing contaminated water.
Clinical Presentation and Disease Burden
The symptoms of PAM can be vague and easy to dismiss in their early stages. "The symptoms of meningoencephalitis are usually fever, headaches, nausea, vomiting and confusion. If it progresses without awareness, you can have hallucinations and seizures with the increased brain swelling," Dr. Cardona said.
The disease can progress rapidly, with confusion, hallucinations, and seizures giving way to coma and ultimately death. Historically, around 97% of those infected with the microbe have died.
Despite its severity, the disease is incredibly rare. Since it was first described around 60 years ago, there have been approximately 500 documented cases. Many people who are exposed to the microbe do not become ill—a study of young people in northwestern Mexico, where the amoeba is endemic, found that 98% of them had antibodies against the amoeba, suggesting that many had encountered it without developing the disease.
Prevention and Early Treatment
Early treatment is critical, and Dr. Cardona urges that people seek medical care right away if symptoms develop after possible exposure. Preventive measures include holding the nose when jumping into fresh water and avoiding putting the head underwater in hot springs. The US Centers for Disease Control and Prevention recommends holding the nose or wearing a nose clip when jumping into warm freshwater.
Emerging Treatment Advances
There are also treatments. In 2025, an unusually large outbreak occurred in Kerala, India. Around 200 people with PAM were treated with anti-parasitic and anti-fungal drugs (搜索). More than half of those whose outcomes were known survived—a striking improvement on the historical death rate of roughly 97%.
This outcome represents a notable shift in the clinical outlook for a disease long considered almost universally fatal, though the rarity of cases and the rapid progression of infection continue to underscore the importance of prevention and early recognition.
