Planetary Health Diet Linked to 28% Lower Cardiovascular Risk in Postmenopausal Women

A 20-year observational study of nearly 67,000 postmenopausal women found that high adherence to the Planetary Health Diet was associated with a 28% lower risk of cardiovascular disease (搜索).

A new analysis from the Women's Health Initiative (WHI) suggests that postmenopausal women who adhere more closely to the Planetary Health Diet may experience substantially lower rates of cardiovascular disease (搜索), coronary heart disease (搜索), stroke (搜索), and heart failure (搜索). The findings were presented at NUTRITION 2026, the annual meeting of the American Society for Nutrition, held July 25–28 in National Harbor, Maryland.
Researchers followed 66,892 postmenopausal women free of cardiovascular disease (搜索) at baseline (1994–1998) for approximately 20 years. Using food frequency questionnaires, the team calculated a Planetary Health Diet Index score for each participant on a scale of 0 to 140, with scores ranging from 35 to 123 across the cohort. Women were then divided into five groups based on adherence levels.
Women in the highest quintile of adherence demonstrated a 28% lower risk of cardiovascular disease (搜索) compared with those in the lowest quintile. More specifically, they showed a 34% lower risk of coronary heart disease (搜索), a 22% lower risk of stroke (搜索), and a 31% lower risk of heart failure (搜索). The researchers adjusted for sociodemographic characteristics, lifestyle behaviors including smoking status, alcohol intake, and physical activity, as well as energy intake and body mass index.
"Moving toward a more plant-forward, higher-quality diet can be accomplished through small, realistic changes," said Donya Shahamati, a doctoral student in public health at the University of California, Irvine, who presented the findings. "These changes might include filling half the plate with vegetables more often; using avocado or olive oil in place of butter; choosing oatmeal or whole-grain cereal for breakfast; or trying a meatless meal once per week."
The Planetary Health Diet is a science-based dietary pattern designed to promote human health while reducing environmental impact from food production. It emphasizes fruits, vegetables, whole grains, legumes, nuts, and unsaturated fats, while limiting red and processed meats, added sugars, refined grains, and saturated fats. The diet shares common features with the Mediterranean and DASH dietary patterns.
One of the most encouraging findings was that even moderate adherence was associated with meaningful risk reduction. For every 10-point increase in the Planetary Health Diet Index score, researchers observed a statistically significant decrease in both cardiovascular disease (搜索) and coronary heart disease (搜索) risk.
"Our results suggest that people may not need to follow a perfect or very restrictive diet to see potential heart-health benefits," Shahamati noted. "This is especially important for postmenopausal women, because cardiovascular risk increases with age and tends to rise after menopause."
Thomas M. Holland, MD, MS, a physician-scientist and assistant professor at the RUSH Institute for Healthy Aging, RUSH University (搜索), who was not involved in the study, commented: "Research focused specifically on older women is particularly valuable because cardiovascular risk increases after menopause, as age-related changes in cholesterol, blood pressure, and metabolism accelerate vascular disease. Studies like this demonstrate that dietary modification remains a powerful tool for improving heart health, even later in life."
Eamon Laird, PhD, lecturer in Nutrition at the Atlantic Technological University Sligo and Adjunct Professor at Trinity College Dublin, who was also not involved in the study, urged caution: "I would add caution as it is only an abstract and would like to see the full detailed analysis before we fully embrace all the findings. Plus because this is an observational study, we cannot conclude that the diet itself caused the lower risk."
The research team is now exploring additional aspects of diet and health in older adult populations, including how diet quality changes over time, which could inform strategies to support healthy aging and prevent chronic diseases. The findings have not yet appeared in a peer-reviewed journal.
