The research, conducted by investigators from the University of South Australia and Flinders University, found that proton pump inhibitors (搜索) (PPIs) used to treat heartburn (搜索) and acid reflux (搜索) significantly impacted both survival and treatment tolerability.
Proton Pump Inhibitors Show Strongest Negative Impact
The analysis of 19 breast cancer (搜索) clinical trials involving 23,211 patients demonstrated that concomitant proton pump inhibitor use was associated with poorer overall survival (hazard ratio = 1.19; 95% confidence interval = 1.08–1.30) and progression-free survival (HR = 1.11; 95% CI = 1.02–1.21). Most notably, patients taking PPIs experienced a 36% increased risk of grade 3 or higher adverse events (odds ratio = 1.36; 95% CI = 1.21–1.53).
"Many women with breast cancer (搜索) are also managing other chronic conditions such as high blood pressure (搜索), diabetes (搜索), or acid reflux (搜索), meaning they are often taking multiple drugs at once," explained lead author Dr. Natansh Modi from UniSA and Flinders University. The researchers believe PPIs might interfere with immune system responses or alter how cancer therapeutics are absorbed and metabolized, though these mechanisms remain poorly understood.
Cardiovascular Medications Increase Treatment Toxicity

Some Medications Show Neutral Effects
In contrast to the concerning findings with PPIs and cardiovascular drugs, cholesterol-managing statins (搜索) and the type 2 diabetes (搜索) medication metformin demonstrated no meaningful impact on survival or treatment-related side effects. This finding aligns with previous research from 2021 that associated statins with positive outcomes in breast cancer (搜索) patients.
Clinical Implications and Recommendations
The researchers emphasize that their findings should not prompt patients to discontinue necessary medications without medical supervision. "Our results don't suggest that people should stop taking their non-cancer medicines, but it underlines how important it is for doctors to regularly review patient medications because people are living longer and managing multiple health issues," Modi stated.
Corresponding senior author Ashley Hopkins, an associate professor at Flinders University, specifically addressed the PPI findings: "It doesn't mean that patients should cease their reflux medication without medical advice, but clinicians should be alert to potential risks and review whether PPIs are genuinely needed."
