The trial, conducted through the FRESH (Females Rising through Education, Support and Health) program in Umlazi township, KwaZulu-Natal, represents a significant milestone in addressing the geographic and demographic gaps in HIV (搜索) cure research. According to researchers, 67% of the world's 40.8 million people living with HIV are in Africa, yet almost all previous cure trials have taken place in the United States.
Revolutionary "Kick and Kill" Approach
The clinical trial, sponsored by California-based drugmaker Gilead Sciences and launched in 2022, employed a novel "kick and kill" strategy to target HIV (搜索) hiding in cellular reservoirs. Twenty women who had been on antiretroviral therapy (搜索) for an average of seven years were enrolled in the study.
The treatment protocol used vesatolimod, a unique drug designed to flush HIV (搜索) out of its hiding places in lymph nodes, liver, and brain tissue. This was followed by a one-time infusion of broadly neutralizing antibodies (bNAbs) on day seven, specifically tailored to target clade C - the world's most common HIV subtype prevalent in sub-Saharan Africa.
"HIV (搜索) doesn't live in our blood, it lives in reservoirs whether in the lymph nodes or the liver or the brain, so you need to kick it out, so that the bNAbs can reach it," explained Dr. Krista Dong, assistant professor at the Ragon Institute (搜索) of Mass General Brigham, MIT and Harvard, and FRESH's clinical director. "So what we were going for was this strategy of kick and kill."
Over 20 weeks, participants received vesatolimod biweekly. On day 35, they discontinued their antiretroviral medications under extremely close monitoring - a critical step that carries real risks but remains the only way to assess whether patients achieve true remission.
Promising Results and Ongoing Success
The trial yielded remarkable results: while 16 participants experienced viral rebound within a year and resumed treatment, four women remained in remission with undetectable viral loads. This 20% success rate represents a significant achievement in HIV (搜索) cure research.
Of the four women who initially remained in remission, one later experienced viral rebound, and two others chose to resume antiretrovirals - one for job-related convenience and another to ensure pregnancy safety. However, one participant, Anele, remains HIV (搜索)-free more than two years after stopping medication.
"Even though the FRESH trial was not a home run, achieving long-term viral control in 20% of the participants is exciting," Dong said. "It has brought hope to the community that progress is being made."
Addressing Critical Research Gaps
The trial addresses several crucial gaps in HIV (搜索) cure research. Professor Thumbi Ndung'u from the University of KwaZulu-Natal, who led the laboratory work, emphasized the importance of conducting trials in African populations where the epidemic's characteristics differ significantly from Western countries.
"In the West, the epidemic is predominantly in white, homosexual men, while in Africa, the majority of people living with HIV (搜索) tend to be younger and female," Ndung'u explained. The biological differences matter significantly, as responses to HIV vary between men and women due to sex hormone influences.
Women represent 53% of HIV (搜索) infections globally but fewer than 19% of enrollees in HIV cure trials, highlighting a critical underrepresentation. The viral biology also varies by region, with sub-Saharan Africa predominantly affected by clades A, C, and D, while North America and Europe see mainly clade B infections.
Future Research Directions
Researchers are now investigating what distinguished the four women who achieved remission. Ndung'u suspects genetic factors may have enabled more effective immune responses, while Dong is examining whether larger viral reservoirs or negative immune reactions to the drugs contributed to treatment failure in other participants.
The team plans to expand their research to include women infected at unknown times, representing more typical HIV (搜索) populations. There's also growing interest in incorporating participants with co-infections common in sub-Saharan Africa, such as tuberculosis (搜索) or hepatitis B (搜索).
"That's a unique but important challenge," noted Helena Lamptey, an immunology researcher at the University of Ghana. "Many people with HIV (搜索) have co-infections which are impacting the immune system and possibly helping HIV to persist."
The success has sparked regional interest, with researchers in countries like Botswana expressing desire to conduct their own trials. Combined with emerging treatment possibilities including therapeutic vaccines and advanced "super bNAbs," there is genuine optimism about turning a corner for HIV (搜索) treatment in Africa.
The FRESH program's unique approach, which combines social empowerment with medical intervention, enabled early detection and treatment that may have contributed to the trial's success. By offering twice-weekly HIV (搜索) testing alongside education and career mentoring in a community where more than 60% of young women contract HIV before age 23, the program created optimal conditions for cure research while addressing broader socioeconomic factors.
