Step Volume or Step Pace? Large Cohort Study Finds Both Walking Strategies Lower Mortality Risk

A University of Sydney-led analysis of more than 64,000 middle-aged adults found that varied step counts and walking intensities were each linked to lower mortality risk.
Walking fewer than 5,000 steps daily at a brisk pace of about 80 steps per minute produced mortality reductions comparable to 5,000-7,500 slower steps.

Adults do not need to reach 10,000 daily steps to reduce their risk of premature death, according to a large prospective cohort study published in the British Journal of Sports Medicine. The analysis found that different combinations of step volume and stepping intensity were associated with lower all-cause and cardiovascular disease (搜索) mortality, suggesting that people facing barriers to walking more can instead walk faster.
The research was led by a team from the University of Sydney, which analyzed records from more than 64,000 middle-aged individuals in a large health research database, following them for an average of eight years. Step counts and stepping intensity were cross-referenced against deaths and cause of death across the study period. A separate description of the same work reports that movement data came from more than 100,000 adults in the UK Biobank cohort who wore wrist activity trackers for seven consecutive days between 2013 and 2015, with deaths tracked for about eight years.
Beyond the 10,000-Step Target
The study's central message is that the widely cited 10,000-step goal lacks a strong evidentiary basis. "That heavily quoted step target actually came from a 1960s Japanese pedometer campaign rather than from evidence," said behavioral scientist Nicholas Koemel of Monash University. "The important message is that every step counts, and how you take those steps can also matter."
The researchers framed their work as filling a gap in the literature. "Most prior step-related studies focused on intensity and amount in isolation, resulting in a paucity of evidence understanding the joint relationship between volume and intensity of total daily steps," they wrote. "Our study, using a large prospective cohort coupled with an advanced accelerometry algorithm to identify steps, examined different step-based strategies associated with all-cause mortality (ACM) and cardiovascular disease (搜索) (CVD) mortality risk."
Participants were sorted into four groups: fewer than 5,000 steps per day, 5,000-7,500 steps, 7,500-10,000 steps, and more than 10,000 steps. Intensity was assessed using peak 30-minute cadence — the average steps per minute during the 30 highest-cadence minutes of each day, which did not have to be consecutive.
Comparable Benefits From Fewer Fast Steps or More Slow Steps
The analysis identified a similar and substantial reduction in mortality risk for two distinct groups: those walking fewer than 5,000 steps per day but walking briskly at around 80 steps per minute, and those walking 5,000-7,500 steps per day at a slower pace of around 60 steps per minute. In practical terms, individuals without time to accumulate 7,500 steps may gain comparable benefit by walking with greater intensity, while those unable to walk quickly may benefit from accumulating more steps.
Reported mortality risk reductions by group were 28 percent for those taking fewer than 5,000 steps daily at a peak cadence of approximately 80 steps per minute, 30 percent for 5,000-7,500 steps at roughly 60 steps per minute, and 43 percent for 7,500-10,000 steps at about 100 steps per minute.
Reaching a peak 30-minute cadence of around 90-100 steps per minute was identified as something of a sweet spot and a potentially useful health target. Setting pace aside, 7,500-10,000 steps per day was linked to the lowest mortality risk.
"If you can't rack up steps, pace matters," said study coauthor Borja del Pozo Cruz, associate professor at Universidad Europa (搜索) in Madrid. "So someone with a 15-minute lunch walk isn't out of options: Walking briskly during that window may do more for them than the same duration at a stroll."
Matthew Ahmadi, associate professor at Monash University in Melbourne and a coauthor, said the importance of pace among people taking fewer than 5,000 steps per day was not entirely surprising based on the team's previous research, though the strength of the association was more pronounced than expected. "It suggests intensity is doing a lot of the 'work' for people who are otherwise quite sedentary," he said.
Independent Experts Urge Caution on Interpretation
Amanda Paluch, associate professor of kinesiology at the University of Massachusetts Amherst, who was not involved in the study, described the findings as encouraging for less active individuals. "If you're among the least active, the biggest bang for your buck comes from walking your existing steps a little faster and working up toward a few thousand more," she said. "This shows that you don't need to overhaul your routine to get a real benefit."
Paluch nonetheless cautioned against a blanket prescription. "The story isn't 'walk faster, always,'" she said. "It's that cadence matters most when you're doing very little, and once you're accumulating a solid volume of steps, how fast you take them becomes secondary."
I-Min Lee, professor of medicine at Harvard Medical School and professor of epidemiology at the Harvard T.H. Chan School of Public Health, emphasized that the evidence base on step intensity remains unsettled. "We rely on the totality of evidence for scientific knowledge, and not on a single study," she said. "While this study showed that fewer faster steps were associated with comparable benefits as more steps at a slower pace, other studies have not supported this."
Lee also cautioned against treating 90-100 steps per minute as an established recommendation, noting that research on step intensity is relatively new and no accepted standard measure yet exists. "I think focusing on walking more, or walking faster, or both are all good options," she said. "One should do what one is capable of, and will do consistently."
Mechanistic Rationale and Prior Evidence
Previous research has consistently linked higher daily step counts with lower premature death risk. A recent systematic review and meta-analysis found that 7,000 steps per day was associated with a 47 percent lower risk of death from any cause compared with 2,000 steps. Evidence on whether step intensity provides benefits beyond total volume has been less consistent, and the new study adds to that knowledge by examining combinations of volume and intensity.
Paluch suggested a physiological explanation for the pace effect: increasing movement intensity places greater demands on the heart and lungs. "This drives the demand on our body in a positive way by increasing heart rate and oxygen consumption, which is the training stimulus that improves fitness over time," she said. Cardiorespiratory fitness is another mechanism that could help explain the findings, since the same walking pace may represent greater relative effort for someone who is less active than for someone already fit — although the study investigators did not measure fitness, limiting conclusions on this point.
Study Limitations
The observational design means cause and effect cannot be proven; the researchers observed associations rather than testing scenarios against one another. Step activity was recorded for only part of the study period and may have changed over time. Although multiple factors including diet and age were accounted for, other hidden influences — such as illness or social isolation — may have affected both step count and mortality risk.
The researchers attempted to reduce reverse causation by excluding people with prior cardiovascular disease (搜索) or cancer (搜索) and deaths within the first two years of follow-up, but this cannot be completely ruled out. Paluch noted that a person's ability to walk quickly may itself signal better underlying health. "People who move well are able to walk faster, and they're also less likely to die early for reasons that may have nothing to do with the walking itself," she said.
Activity was measured for only seven days, which may not reflect overall habits, and wrist trackers can occasionally miss steps when the wrist is not moving, such as when someone is pushing a shopping cart, Ahmadi noted. A strength of the study, however, was objective measurement of movement with wearable devices rather than reliance on participant-reported activity.
Practical Guidance
Ahmadi advised against fixating on cadence numbers. "Don't overthink the cadence number," Paluch said. "Ninety to 100 steps per minute is roughly what a brisk walk feels like." Ahmadi suggested aiming for a pace at which a person can still talk but doing so takes some effort, consistent with current recommendations for at least 150 minutes per week of moderate-intensity aerobic activity such as brisk walking.
Those faster steps need not occur in one continuous session, Ahmadi said; the findings are consistent with accumulating short bursts of movement throughout the day, such as walking briskly to the bus, taking the stairs, or picking up the pace between errands.
"There's no 'one size fits all' approach to lower mortality risk through walking," Ahmadi said. "Someone can choose a higher number of steps at a comfortable pace, or fewer steps more briskly, and both are legitimate, evidence-based paths. That's particularly encouraging for people who face real barriers to walking a lot, such as having limited time availability."
The researchers concluded that their findings contribute to the evidence base for step-based approaches targeting both daily step counts and stepping intensity, and may inform interventions for middle-aged and older adults, particularly those with barriers to increasing daily step counts or stepping intensity. "Our study moves beyond single-metric approaches (eg, focusing solely on daily step counts) to examine combinations of different step dimensions in relation to mortality risk, informing future step-related public health and clinical guidelines," they wrote.
