Targeted Electroacupuncture Shows Promise for Cognitive Symptoms in Breast Cancer Survivors

A randomized, double-blind pilot clinical trial led by researchers at the University of California, Irvine has demonstrated that targeted electroacupuncture (搜索) may offer significant benefits for breast cancer (搜索) survivors struggling with persistent cognitive and psychological symptoms. The study, published in the Journal of the National Cancer Institute, provides evidence for a precision-based approach to integrative cancer care that could address the long-term neuropsychiatric challenges faced by millions of survivors.
Study Design and Methodology
The 10-week trial enrolled 35 breast cancer (搜索) survivors who self-reported symptoms of cognitive impairment (搜索), fatigue (搜索), insomnia (搜索), and psychological distress (搜索). Participants were randomly assigned to receive weekly electroacupuncture (搜索) treatments targeting either neuropsychiatric-specific acupoints (搜索) or nonspecific control acupoints. The comprehensive evaluation included neurocognitive testing, patient-reported outcomes, plasma biomarkers, and neuroimaging to assess treatment effects and underlying mechanisms.
"Our randomized, controlled trial incorporated multiple aspects, including cognitive testing, brain imaging, and blood-based biomarkers, to understand the mechanisms underlying our intervention," said corresponding author Alexandre Chan, professor and founding chair of the Department of Clinical Pharmacy Practice at UC Irvine (搜索)'s School of Pharmacy & Pharmaceutical Sciences.
Significant Clinical Improvements
Participants receiving targeted electroacupuncture (搜索) showed substantial improvements in key outcome measures. The neuropsychiatric-specific acupoints (搜索) group demonstrated significant improvements in attention (Glass's Δ = 0.562; adjusted P < .05) and psychological distress (搜索) (Δ = 0.764; adjusted P < .05) at the end of treatment. Notably, these benefits persisted four weeks after treatment completion, with continued significant differences in attention (Δ = 0.708) and distress (Δ = 0.711).
The response rates further highlighted the targeted approach's superiority. Nearly 43% of patients in the neuropsychiatric-specific group experienced measurable cognitive enhancement, compared to only 12.5% in the control group. For psychological distress (搜索), 50% of the targeted treatment group showed improvement versus 37.5% in the nonspecific group.
Neurobiological Evidence
Brain imaging provided compelling evidence of the treatment's biological effects. Participants receiving targeted electroacupuncture (搜索) showed greater gray matter volume compared to the control group (P = .033), with this increase positively correlating with improved attention function (r = 0.69; P = .020).
The study also revealed specific changes in brain network connectivity patterns. Improvements in memory and response speed were associated with reduced connectivity in the Default Mode Network (搜索), which is involved in internal, self-focused mental tasks (r = −0.93; P < .01), and increased connectivity in the Dorsal Attention Network (搜索), which relates to sustained focus and shifting attention (r = 0.86; P < .001).
Additionally, numerous biomarkers related to neuroinflammation (搜索) were reduced following electroacupuncture (搜索) treatment, providing further mechanistic insight into the intervention's effects.
Clinical Significance and Safety
The treatment demonstrated an excellent safety profile, with all reported adverse events being grade 2 or lower. Of the 35 enrolled participants, 30 patients (86%) completed all 10 electroacupuncture (搜索) sessions, indicating good tolerability and adherence.
"These results suggest that where acupuncture is applied matters," Chan explained. "Targeting acupoints linked to specific brain and neurological functions appears to produce measurable changes not only in symptoms but also in their biomarkers and in the brain."
Addressing Unmet Medical Needs
The research addresses a significant clinical challenge in cancer survivorship care. Advances in detection and treatment have pushed breast cancer (搜索) survival rates above 90%, leaving more than 4 million survivors in the United States. However, many continue to face long-term neuropsychiatric symptoms that significantly impact quality of life.
"Patients often report feeling unprepared for the cognitive and emotional challenges that persist after treatment," Chan noted. "We need robust scientific evidence to show how effective interventions can be integrated into their treatment in order to reduce survivors' symptoms and improve their healing journeys."
Future Research Directions
While the findings are promising, the researchers emphasize that these are preliminary results from a pilot study. The team is calling for larger, multicenter trials to confirm the results and better understand the biological mechanisms behind electroacupuncture (搜索)'s effects.
"Current guidelines support the use of acupuncture to manage distress in people with cancer. However, these recommendations often lack precision," Chan said. "It's time to step up and further optimize how integrative therapies are delivered to patients."
If validated in larger studies, this targeted approach could help establish standardized, evidence-based acupuncture protocols for cancer survivorship care, offering patients a non-pharmacological treatment option that avoids the risks of side effects, dependency, and interactions with ongoing cancer therapies.
