Three Daily Servings of Full-Fat Dairy Show No Adverse Metabolic Effects in Overweight Adults, Study Finds

A randomized controlled trial led by University of Toronto scientist G. The results, published in The Journal of Nutrition, challenge decades of dietary guidance favoring low-fat and fat-free dairy products.
The study enrolled 74 participants classified as overweight (搜索) or obese and randomly divided them into three groups. One group followed a low-dairy, calorie-restricted diet. A second consumed three daily servings of dairy while keeping calorie intake "energy neutral," meaning participants ate roughly the amount of energy their bodies used. A third group also consumed three daily servings of full-fat dairy but did not face calorie limits. All participants were encouraged to follow Canada's food guide, and the design allowed researchers to separate the effects of dairy from those of calorie restriction.
After 12 weeks, participants consuming three servings of dairy showed no meaningful differences in weight gain, body composition, or cholesterol compared with those eating the low-dairy diet. "Those who had three servings of dairy didn't have adverse levels of blood cholesterol or lipids or evidence of insulin resistance," said Anderson, a professor of nutritional sciences in U of T's Temerty Faculty of Medicine. Insulin resistance occurs when the body's cells stop responding properly to insulin, the hormone that helps move sugar from the bloodstream into cells for energy; over time, that reduced response can increase the risk of type 2 diabetes (搜索).
The higher dairy groups also recorded lower blood pressure and consumed more calcium, protein, and vitamin D, nutrients that support functions such as bone maintenance, muscle health, and immune activity.
The Dairy Matrix Hypothesis
The apparent contradiction between these findings and long-standing low-fat guidance may stem from viewing saturated fat in isolation rather than considering the food that contains it. Anderson and other researchers point to the dairy matrix hypothesis, which proposes that a food's physical structure changes how its nutrients are broken down, absorbed, and used.
"With dairy products, it's got two proteins – casein and whey – that are bound together with fat and with nutrients mixed in," Anderson said, adding that this unique and complex physical structure is responsible for the slow and steady delivery of nutrients from dairy products, expanding their health impacts beyond the sum of the isolated nutrients.
Jessica Cording, R.D., author of The Little Book of Game-Changers, noted that the dairy matrix is coming up more frequently among food scientists, but "the jury's still out. Further research may show us more about the dairy matrix and its impact on health."
Conflicting Evidence and Study Limitations
Nutrition guidelines in Canada and many other countries have long favored fat-free or low-fat dairy, reasoning that reducing saturated fat may help prevent high cholesterol and lower cardiovascular disease risk. However, human research has not consistently shown that eating full-fat dairy leads to worse health outcomes, Anderson said. Some investigations have even identified possible protective effects, including recent work led by Kozeta Miliku, an assistant professor at Temerty Medicine.
The study authors acknowledge important limitations. The trial did not directly compare full-fat dairy with low-fat dairy—it only compared full-fat dairy with a low-dairy diet. It is also possible that the 12-week study was too short to detect meaningful increases in weight, cholesterol, and insulin resistance.
Experts also cautioned against attributing the observed blood pressure improvements solely to dairy. "Blood pressure is influenced by someone's entire diet pattern, so it's difficult to attribute the change in blood pressure observed in this study to milk fat alone," Cording said.
Implications for Older Adults and Dietary Guidance
The findings may be particularly useful for older adults, Anderson noted. As people age, their overall energy needs often decline even though they still require adequate protein, calcium, vitamin D, and other nutrients. That creates a practical challenge: older adults may need to obtain more nutrition from fewer calories. Dairy products offer a familiar source of concentrated energy and nutrients, and the study found no evidence that including them increased the risk of diabetes or other chronic diseases.
The results also support a broader shift in nutrition research. Instead of judging foods mainly by a single component such as saturated fat, researchers and policymakers may need to consider how nutrients behave when consumed together in a whole food.
The American Heart Association continues to recommend low-fat and fat-free dairy products because they are lower in fat, saturated fat, cholesterol, and calories, noting that saturated fats can raise levels of LDL ("bad") cholesterol in the blood and increase the risk of heart disease and stroke. Anderson acknowledged that age should factor into the decision: "Children need more energy. Having higher fat—which is calories—is absolutely great for them. For somebody older who has already accumulated quite a bit of fat, you might be better off choosing low-fat dairy."
Ultimately, Cording emphasized that "full-fat dairy isn't automatically unhealthy, just as low-fat dairy isn't automatically healthier. Nutrition is rarely about one food. It's really someone's overall diet pattern that matters most for long-term health." Anderson's advice was similarly straightforward: "Keep it simple, eat a variety of foods and not too much of anything."
